    Palliative Care (NCCPPC)
  • The Northern California Collaborative for
    Pediatric Palliative Care (NCCPPC) is a
    collaborative professional group aimed at
    improving services by providing education and
    increasing awareness and advocacy for children
    with life threatening conditions and their
    families. We share information and resources
    related to providing excellent care for children
    with life-threatening conditions and have
    membership from throughout Northern California.

  • The Central California Collaborative for
    Pediatric Palliative Care is a diverse and
    dedicate group of individuals who have come
    together in response to a healthcare crisis
    facing children living in Central California.
    Together we offer support in opening access to
    options for family-centered, compassionate care
    for children with life threatening conditions and
    their families.

    Network (SCPPN)
  • The Southern California Pediatric Palliative Care
    Network (SCPPN) is a collaborative network
    dedicated to improving the quality of life and
    quality of care for children with
    life-threatening conditions by promoting
    excellence in compassionate, family-centered
    palliative care. We meet every other month at
    various locations around Southern California
    working together to ensure that children have
    access to outstanding medical care that meets the
    needs of the entire family.

AB 1745 - The Nick Snow Childrens Hospice and
Palliative Care Act
  • AB 1745 (the Nick Snow Hospice Palliative Care
    Act) was passed by the legislature and signed by
    the Governor last fall. The legislation requires
    the Department of Health Care Services (DHCS) to
    submit a hospice eligibility waiver request that
    would expand eligibility for children and add
    specific palliative care services not currently
    available under the state plan.
  • The waiver was submitted to Federal CMS on May 21

  • Demonstration project to show a better way to
    care for seriously ill children that is fiscally
    responsible and humane
  • 13 counties
  • Year 1 - Santa Cruz, Monterey, Alameda, Santa
    Clara, San Diego (300)
  • Year 2 - Humboldt, Marin, Orange, Sacramento,
    San Francisco and Sonoma (800)
  • Year 3 - Los Angeles and Fresno (1802)

  • IPPC, ELNEC, Regional Support through
    Collaboratives, Conference Calls, Online
    Resources (other needs)
  • Goal of this project is to establish operational
    capacity for community providers to referral
    children appropriately for palliative care, for
    CCS to provide case management and for hospice
    providers to provide the necessary services.
