3Manejo del peso y las comorbilidades
- Está claro que la pérdida de peso reduce los
factores de riesgo cardiovasculares y de diabetes
mellitus tipo 2 - Se asume que esta mejoría ultimadamente se
traduce en una disminución de eventos como
infarto del miocardio. - Hasta hace poco no existían estudios diseñados
prospectivamente que confirmen esta asunción - La diabetes tipo 2 se sabe es causa importante
de morbilidad mortalidad (principalmente
cardiovascular) y la demora en prevenir el
desarrollo de la diabetes tipo 2 puede ser
considerada un evento mayor

vida versus medicación1
- Porqué las modificaciones en el estilo de vida
deben ser la primera elección de tratamiento para
prevenir/retardar la aparición de diabetes? - En el estudio DPP con metformin esta fue menos
efectiva que la modificación del estilo de vida - Los agentes que reducen la Glucosa i.e.
acarbose/metformin - Están asociados con efectos adversos
significativos - Requieren monitoreo
- Están contraindicados en algunos individuos
- No tienen ningún otro beneficio clínico

- Los Pacientes fueron aleatorizados a recibir
tratamiento con Xenical 120 mg ó placebo tres
veces al día diariamente por 4 años en
combinación con cambios en el estilo de vida - Se les prescribió una dieta discretamente
reducida en calorías-para proveer un déficit
calórico diario de 800 Kcal. - A todos los sujetos se les alentó a realizar un
moderado ejercicio físico todos los días - Los pacientes recibieron consejo sobre cambios en
el estilo de vida por 2 semanas durante los

- En este estudio la mayoría de pacientes con
Xenical completaron la fase de tratamiento a
largo plazo comparado con los pacientes en
placebo (52 vs. 34) - La pérdida promedio de peso corporal al año de
seguimiento fue significativamente mayor en el
grupo con Xenical que con placebo y se mantuvo
así hasta el final del estudio - Aproximadamente 50 más pacientes en el grupo
Xenical que con placebo alcanzaron pérdidas de
peso ? 5 y ? 10 tanto al primero como al cuarto
año de tratamiento

- Xenical más cambios en el estilo de vida en 4
años resultó en una mayor pérdida de peso y una
reducción significativa en la incidencia de
diabetes tipo 2 comparado con cambios
significativos en el estilo de vida solamente - Xenical mas cambios en el estilo de vida
resultaron en una reducción significativa y
sostenida en los factores de riesgo
cardiovasculares tales como presión arterial y

- It is clear that weight reduction leads to
improvements in cardiovascular risk factors e.g.
type 2 diabetes - It is assumed that these improvements will
ultimately translate into reductions in hard
outcomes such as myocardial infarction - Until recently there have been no
prospectively-designed studies confirming this
assumption - Since type 2 diabetes is known to cause
significant morbidity mortality (primarily
CVD), the delay or prevention of the development
of type 2 diabetes can be considered a hard
outcome

Xenical 120 mg or placebo three times daily for 4
years in combination with lifestyle changes- - A mildly calorie-reduced diet was prescribed to

- A greater number of Xenical-treated patients
completed the long-term treatment phase of this
study compared with placebo patients (52 vs 34) - Mean weight loss was significantly greater in
Xenical than placebo patients at 1 year and
remained significant at the end of the 4-year

patients achieved weight loss of ? 5 and ? 10

- The hazard ratio showed a 37 decrease with
Xenical compared with placebo - Xenical treated patients also achieved a

4-year study was consistent with that observed in

- Xenical plus lifestyle changes over 4 years
resulted in greater weight loss and a
significantly reduced incidence of type 2
diabetes as compared with intensive lifestyle
changes alone - Xenical plus lifestyle changes resulted in
significant and sustained reductions in
cardiovascular risk factors such as blood
pressure and lipid levels - This study confirms the efficacy and safety of
