What happened PLD HPI - . A 17 ton picker
operator was attempting to extend the boom to
pick up slickline tools. There was not enough
slack on the winch line as the hydraulically
operated boom extended and the ball was pulled
into the boom tip. This resulted in the ball
free falling 55 feet to the rig matting below.
There were no injuries and operations were
suspended.
  • Preliminary Learning Points
  • Never rely on the anti two block system to stop
the picker. Use a spotter if vision is
    obstructed.
  • Always check and verify that the anti two block
system is functioning prior to use.

Re-enactment
