What happened NM/PLD - ROAD TRANSPORT LOAD
CONTACTS AND SEVERS POWER LINE. During a rig
move, a truck hauling a living quarters skid
approached a set of two low-hanging power lines.
The truck was stopped in order to give rig moving
company personnel time to raise the power lines
using fiberglass extension poles. The driver had
been briefed prior to driving under the first
wire about the potential to snag the lines due to
their low height. When the personnel on the side
of the road saw that the truck load would not
clear the second wire, they attempted to stop the
driver with no success, resulting in a severed
power line. The dead end of the line dropped
and struck one employee on the front of his hard
hat and glanced off. The live end of the wire
came to rest on top of the load.

    identified hazards and required controls
    regarding these two low hanging power lines.
  • Poor communications inadequate communications
    were established between the ground spotters and
    the truck driver.
  • Ground personnel were in the line of fire due
    to the need to physically lift the lines in order
    for the load to pass.
  • Inadequate job planning - this particular load
    was not included in the approved Rig Move Journey
    Management Plan/Meetings, as it was dispatched
    from a separate location, using a 3rd party
    hauler.
