BACKGROUND___________
  • ESSP staff initiated reviews of cases receiving
    LTESS funding for individual Supported Employment
    Follow Along Services.
  • The Goals of the Case Reviews were
  • To ensure compliance with requirements as noted
    in the Vendor Agreement and related appendices.
  • To ensure that quality services to mutual
    consumers are planned, provided and documented.

  • To ensure that the billing for follow along
    services accurately reflects the
  • services rendered and documented
  • by our service partners.
  • To ensure communication and collaboration
    between DRS and its service partners.
  • To ensure that appropriate services are
    provided by the service partners and DRS, if
    necessary during long term follow along.

SITE REVIEWS_____________
  • Management of six ESOs volunteered to have LTESS
    cases reviewed.
  • ESSP staff reviewed the cases on site and used a
    protocol based on the Vendor Agreement and
    appropriate appendices to review files and
    document findings.
  • The findings were compiled in formal reports and
    reviewed with management of the ESOs.

LTESS STEERING COMMITTEE REQUEST_________
  • The LTESS Steering Committee asked ESSP staff to
    present and discuss the findings as a way of
    educating ESO staff regarding the strengths and
    weaknesses of long term follow along service
    delivery and documentation of services.

FINDINGS THE GOOD NEWS_
  • In ALL cases, appropriate services were provided
    in a timely manner and subsequently documented.
  • Monthly client contact was provided and
    documented in almost every case.
  • Employment specialists were very consumer
    oriented and responsive to consumer and employer
    needs.

ISSUES/REMINDERS_________
  • DRS counselor approval for LTESS funding must be
    specifically documented.
  • Client stability on the job must be documented
    on the SE Placement and Training form.
  • The Ongoing Services form must be completed in
    every case when required.

ISSUES/REMINDERS (cont.)________
  • Appropriate documentation of the consumers
    disability must be in each case file.
  • If services other than the maintenance of the
    person in employment are needed, the DRS
    counselor should be contacted ASAP.
  • Documentation in the case files of LTFA provided
    should match the hours billed to LTESS. Billing
    should be for actual time spent for service
    delivery, transportation and documentation.
