V 51-58 Admission for Aftercare Management
  • Patient is admitted for the removal of a
    prosthetic device

Aftercare V-Codes
  • Used when treatment for the original illness or
    injury is complete, but . . .
  • The patient requires further care during the
    healing or recovery phase

  • Aftercare codes are not assigned when treatment
    is directed at the acute illness or injury
  • Exception Dialysis, Chemotherapy and Radiotherapy

V57 - Rehabilitation
  • V57 code is sequenced 1st
  • Condition requiring rehab. Is sequenced 2nd

V 67 Admission for Follow-up Examination
  • Pt. has been taking Fen-Phen for past 6 months.
    Pt. stopped drug 2 weeks ago and wants to rule
    out any adverse effects that could be possible
    from the drug.

Follow Up V-Codes
  • Used when patient is admitted for follow-up after
    the initial treatment of a disease or injury has
    been completed.

Follow-up codes include
  • Following surgery
  • Following radiotherapy
  • Following chemotherapy
  • Following psychotherapy, other mental disorder

  • Following treatment of healed fracture
  • Following other treatment
  • High-risk medication
  • Following combined treatment

  • Selection of the follow-up code is based upon the
    patients most recent treatment modality

  • If a recurrence, extension or related condition
    is identified, that condition should be assigned
    as principal diagnosis
  • Do not assign code V67

V 71 Admission for Observation and Evaluation
  • Patient was involved in a car wreck and was
    brought to the hospital for possible head injury.
    No injury was found.

Observation V- Codes
  • Patients without a diagnosis are suspected to be
    at risk for an abnormal condition, but after
    evaluation, neither the suspected condition nor
    an alternative is identified
  • AND

  • No additional evaluation or treatment is required
    at the current level of care.

  • When a related condition is identified, the code
    for that condition is assigned.
  • No V71 code would be assigned.

  • If the patient has symptoms related to the
    suspected condition and the suspected condition
    is ruled out, code the symptoms. Do not code
    V71.

  • V71 is usually assigned as a solo code. Two
    exceptions
  • Chronic conditions requiring care during the
    admission (diabetes, hypertension, COPD, etc.)
  • Minor injuries which would not have required
    inpatient admission

  • Suspected mental disorder

V 66.7 Admission for Palliative Care
  • Used for patients who are in the terminal phase
    of an illness.

  • End-of-life care, hospice care
  • It cannot be used as a principal diagnosis (or it
    cannot be sequenced 1st)

V 73- V82 Screening Exams
  • Patient with family history of breast cancer
    undergoes routine mammography.

Screening V-Codes
  • Screening is performed on patients who present no
    signs or symptoms relative to the disease.

  • If a condition is discovered during screening,
    then the code for the condition may be used as an
    additional diagnosis code.

condition
  • Patient has a personal history of prostate

    under treatment, or if a complication is present,
    do not assign a code from V10 V15

  • Patient has a family history of hypertension.

  • Continuing condition or health status

    influence the patients stay or the health care
    that they receive.
  • Terms status post are used frequently to
    describe these problems
