What are Haemorrhoids?
Haemorrhoids or piles are swollen blood vessels
in or around the anus and rectum. Sometimes these
vessels swell and cause the walls of the veins to
stretch and become thin causing irritation during
bowel movements. Haemorrhoids are more common
among men as compared to women. In most of the
cases, women suffer from haemorrhoids during
pregnancy.
3
Types of Haemorrhoids
Internal haemorrhoids Internal haemorrhoids lie
far inside the rectum and because of the presence
of a few pain sensing nerves inside the rectum,
internal haemorrhoids do not hurt much. Sometimes
these haemorrhoids protrude out of the anal
sphincter, hurting and creating irritation when
rubbed by cloth while walking or sitting. Either
they recede into the rectum on their own or are
gently pushed back into the rectum with the help
of a surgical procedure. External haemorrhoids
External haemorrhoids lie within the anus and are
often uncomfortable. They can be felt or seen if
they prolapse out causing extremely painful
condition if the blood clots from within the
prolapsed external haemorrhoids. This painful
condition is called thrombosis. These thrombosed
haemorrhoids are painful and may cause bleeding.

Signs and Symptoms
  • Bleeding during bowel movement, dripping into
the toilet bowl or staining on toilet paper.
  • Protrusion during bowel movement.
  • Pain or itching in the anal area.
  • Anal lumps.

5
Treatment
If one witnesses these signs and symptoms, he or
she should undergo a treatment to cure
haemorrhoids. The mild symptoms can be relieved
frequently by improving and regularizing bowel
habits. Simple treatments such as injection or
tying with a special rubber band cures piles
sometimes. If these methods dont work then
surgical methods are the last options to cure
haemorrhoids.

Surgical Treatments
1) Open Haemorrhoidectomy This method is best
for third or fourth degree piles. The method
removes the piles by cutting them out. It uses
anesthesia and may require hospitalization for a
short duration. The healing of wounds takes 6-8
weeks. Pain and bleeding in postoperative period
are the main side effects. 2) Laser
Haemorrhoidectomy This surgery uses laser to
treat haemorrhoid but it has no advantage over
standard operative techniques rather making it an
expensive procedure.
7
3) Stapler Haemorrhoidectomy This surgery aims
to minimize the pain and improve the healing
properties. This surgery removes a cuff of a
tissue from the area just above the piles,
pulling the piles back into its normal positions
within the anal canal. The stapler also
interrupts the blood supply to the piles causing
them to shrink in size and stop bleeding. The
wound usually heals within 1-2 weeks with little
pain.

4) Transanal Haemorrhoidal Dearterialization
(THD) THD employs a specially designed
proctoscope coupled with a Doppler transducer to
allow identification and suture ligation of the
haemorrhoidal arteries. This is suitable for
patients who have bleeding with no prolapsed
haemorrhoids. Post surgery pain in case of this
surgery is less than that of the stapler
haemorrhoidectomy procedure. 5) Rubber band
ligation In this method, a rubber band is tied
around the haemorrhoid to cut off the circulation
resulting in the shriveling and dropping of the
haemorrhoid.
