    implement sustain evidence-based health

    CMS for Beneficiary Outreach Assistance

    HICAP, AAAs and ADRCs
  • Funds enhanced outreach for Medicare
    Beneficiaries focusing on Medicare Part D
  • People Eligible for Low Income Subsidy Program
    (LIS) and Medical Savings Program (MSP)
  • Beneficiaries residing in rural areas

  • EBHP Grant ADRCs are an important link for
    access to information about health promotion and
    disease prevention programs
  • Alzheimers Grant One grant enabled Alzheimers
    Association chapters to provided training to ADRC
    staff in San Diego and Del Norte counties.
    Unfortunately, funding reduction limited
    expansion to other ADRCs.
