  • When their patients sent to SMGH for Cardiac
    Cath, issues with lack of documentation
  • SMGH arranged the Coordinator of the Cath Lab to
    present
  • Coders explained their issues
  • Form was revised to meet their needs

  • Smaller hospitals coders code everything
