- The College is pleased to offer you a
comprehensive Benefits Program. As New Jersey
State employees, we enjoy the Benefits that are
offered to all New Jersey State employees through
the New Jersey Division of Pensions and Benefits
and the State Health Benefits Program.
3When are my Benefits Effective?
- 10 month employees hired with a 9/1/10 contract
start date will have health benefits effective
9/1/10. - All other 10 month employees and all 12 month
employees will have health benefits effective 2
months from their start date. - Eligible part time employees may purchase the
health benefits and they will be effective 2
months from their start date.
4Enrollment of Family Members
-
- With proper documentation, an eligible employee
may enroll his/her spouse, domestic partner,
civil union partner, children, step-children and
adopted children in the health, prescription and
dental insurances. - Children can be covered under the employees
insurances until December 31st of their 26th
birthday.
5 Health Insurance Options
- NJ Direct15 is a Preferred Provider
Organization (PPO). NJ Direct15 is administered
through Horizon Blue Cross Blue Shield of New
Jersey. - Two HMOs are available Aetna HMO and Cigna
Healthcare.

In-network providers are available nationwide through NJ Direct15.
You do not have to select a primary care physician.
You do not need a referral to see a specialist.
7In-network Services for NJ Direct15
- If a doctor participates in the Horizon BCBSNJ
managed care network, members will pay the usual
co-payment for eligible services. In 2011, the
co-pay is 15.00 for office visits. - Outside of New Jersey, members can utilize
doctors who participate in the national Blue
Cross Blue Shield Managed Care Network. - To verify in-network coverage, check with the
doctor and/or the Provider Directory on the NJ
Direct15 web site.

12Are Aetna HMO and the Cigna Healthcare Plans the
same HMOs of the past?

- Aetna and Cigna Health care are HMOs.
- You select a primary care doctor when you enroll.
- You get a referral (most are electronic) to see a
specialist. - You cannot go out of network.
- You have no deductibles or claim forms to
complete. - You pay the usual co-pay (15.00 for office
visits during 2011) to see your PCP or referred
specialist. - Both HMOs are available nationwide.

14Plan Costs
-
- Most State employees pay 1.5 of their base
wages on a biweekly basis for the SHBP medical
plan and/or prescription drug coverage. - Regardless of the plan or level of coverage, the
payroll contribution will be the same 1.5.
15Prescription Drug Coverage
- The prescription drug coverage is administered
through Medco. An employee who is paying the 1.5
payroll deduction for the health will not have an
additional payroll deduction for the
prescription. Employees who do not sign up for
health but only sign up for prescription, must
pay 1.5 of their base wages for the Prescription
Drug Coverage. - The Prescription Drug Program covers the cost of
prescription drugs for employees and eligible
dependents for use outside of hospitals, nursing
homes, or other institutions. -

17Full Time Employee Enrollment Application
An employee who enrolls as member and
spouse/partner must provide a copy of his/her
marriage certificate or civil union certificate
and a copy of the federal tax return from the
previous year. An employee who enrolls as
family, must provide a copy of his/her marriage
certificate or civil union certificate and a copy
of the federal tax return from the previous year
plus a copy of the birth certificate for each
child. If the child has a different last name, an
Affidavit of Dependency must also be completed.
If the children are adopted, legal paperwork
and an Affidavit of Dependency must be provided.
An Employee who enrolls as parent/child(ren)
must provide a copy of the childs birth
certificate and complete an Affidavit of
Dependency . Notarization of the Affidavit of
Dependency form can be completed in human
resources.
18Full Time Employee Enrollment Application Cont.
A New employee must complete the insurance
application within 60 days of his/her hire date.
In order to be timely with the State Health
Benefits Division, new hires should submit the
application to the Benefits Administrator in the
Office of Human Resources within the first 14
days of their hire date. For new employees, the
payroll deduction for the health insurance plan
will begin in the payroll that coincides with the
coverage effective date. The Health and

19How to Waive Health Insurance
- Employee completes state waiver form and state
health insurance application certifying that
he/she has other health insurance coverage. - Employee must submit both forms to avoid the
1.5 contribution. - The deadline to complete the waiver form and
health insurance application is 2 months after
your hire date. - The forms are available on the HR website at

20Part Time Employee Health and Prescription
Insurances
- Active part-time employees who participate in
a New Jersey State-administered pension plan are
eligible to enroll in SHBPs New Jersey Direct15
Medical Plan and the Employee Prescription Drug
Plan. - Dental insurance is not available under the part
time employee insurance program.

24Dental Insurance
-
- The State Employee Group Dental Program is
available to full-time State employees and their
eligible dependents. - The College offers two types of dental plans, a
Dental Expense Plan and several Dental Plan
Organizations (DPO). - Both types of plans pay the full cost of
cleaning and exams which can be done twice per
year (every six months). -

25- Dental Expense Plan
- The Dental Expense Plan is a traditional
indemnity- type plan administered by Aetna
Dental. The plan allows employees to choose any
licensed dentist for dental care. If the dentist
doesnt accept dental insurance, you will pay the
dentist and then submit a claim form for
reimbursement. - The annual plan deductible is 50 per person
150 per family. The deductible does not apply to
diagnostic, preventive, and orthodontic services.
After the annual deductible is satisfied,
reimbursement is a percentage of the reasonable
and customary charges for services that are
covered under the plan.
26- Dental Plan Organizations
-
- The Dental Plan Organizations (DPOs) are
companies that contract with a network of
providers for dental services. You must use
providers participating with the DPO you select
to receive coverage. There is no annual
deductible with the DPOs and you will usually pay
a co-pay for services such as fillings, crowns
etc. There are several DPOs to choose from in the
State Group Dental Program including the

32COBRA Upon Termination

- If an employee has been enrolled in health
insurance and he/she terminates employment with
The College, he/she will be eligible for coverage
through the COBRA program. This will allow the
employee to purchase the insurance coverage at
the group rate for up to 18 months. - COBRA paperwork is automatically sent upon
receipt of the employees termination notice. -

34Vision Care Reimbursement
- Full time employees may be eligible for a Vision
Care Reimbursement. Eye exams are covered under
the health insurance coverage. Employees may have
their eyes examined once a calendar year for the
15.00 co-pay. -
- In addition, all full-time employees and their
dependents are entitled to the following maximum
reimbursement under the vision care plan. - Regular Lens Bifocal

only one payment for glasses and one payment for
an examination during the period July 1, 2009 to

35Mandatory Retirement Programs
- The College of New Jersey participates in the
State of New Jersey public employee pension
programs the Public Employees Retirement System
(PERS) and the Alternate Benefit Program (ABP). -
- All permanent employees are eligible for
enrollment on first day of employment. - Temporary employees are eligible for enrollment
after one year of employment.
36Classified Employees
- The Public Employees Retirement System (PERS) is
a defined benefit program open to permanent,
classified, full time employees. - Employees are required to contribute 5.5 of
their gross bi-weekly salary. - Employees are vested after 10 years of service or
at age 62. - Employees will have life insurance at 3 times
their annual salary. The employee will pay .5
of their bi-weekly salary for the life insurance. - After 3 years of service, the employee is
eligible for the pension loan program.
37Classified Employees
A New employee must complete and return the PERS
enrollment application and PERS Beneficiary form
to the Pension Administrator in the Office of
Human Resources within 5 days of his/her hire
date. For new employees, the payroll deduction
for the PERS plan will be retro-active to their
hire date. Please note that the PERS application
must be first approved and certified by the State
of New Jersey Division of Pensions and

38Faculty and Non-Unit Staff Employees
- The Alternate Benefit Program (ABP) is a
tax-sheltered, defined contribution retirement
program for higher education faculty and certain
administrators. - Full-time and adjunct faculty, part-time
instructors, visiting professors and
administrative staff are required to possess a 4
year college degree or its equivalent to
participate in the ABP. - Employees will have life insurance at 3.5 times
their annual salary and long term disability
coverage after one year of active service in ABP. -
- Full time employees without a 4 year college
degree will be enrolled in PERS.

- The Additional Contributions Tax-Sheltered
(ACTS) Program allows eligible employees to
obtain supplemental tax-deferred annuities with a
variety of carriers through a salary reduction

Compensation Plan (NJSEDCP) provides eligible

Internal Revenue Code Section 457 and the laws of
the State of New Jersey, is administered by

44Flexible Spending Accounts
- The State Employees Tax Savings Program
Taxave, is a benefit program available under
Section 125 of the Federal Internal Revenue Code
that allows eligible employees to set aside
before-tax dollars to pay for certain medical,
dental, and dependent care expenses, thereby
avoiding federal taxes and saving money. - An eligible employee is any full-time employee
of the state, a state college or university, or
other state agency who is eligible to participate
in the State Health Benefits Program.
45- Pretax Payroll Deduction Option
- Employee payroll deductions for Health and
Dental Insurances can be taken on a pre-tax basis
-this is the default. If you want to pay tax on
the deductions, you can complete a form so that
the deductions will be taxed. - Dependent Care Spending Account
-
- The Dependent Care Flexible Spending Account
(Dependent FSA) allows an employee to set aside
funds to pay for anticipated expenses related to
dependent care required to permit the employee
and spouse to work. Reenrollment is required each
calendar year.
46Un-reimbursed Medical Spending Account
- The Unreimbursed Medical Flexible Spending
Account (Medical FSA) allows employees to set
aside money to pay for qualified medical and
dental expenses not paid by any group benefits
plan under which they or their dependents are
covered. Reenrollment is required each calendar
year. - Transportation/Commuter Tax Save
- The New Jersey State Employees Commuter Tax
Savings Program allows eligible employees to set
aside before-tax dollars to pay for mass transit
and commuter parking expenses, thereby avoiding
federal taxes and saving money.

48Long Term Care
- Enrollment in the Long Term Care Insurance Plan
is available to individuals and their eligible
family members who are employed by The College.
The Prudential Insurance Company of America
administers the Plan. The Plan pays a daily
benefit amount toward the cost of care in the
home and community-home health care, adult day
care, informal care, and care that is provided in
licensed facilities such as assisted living and
nursing homes. - New employees who enroll within 60 days of hire

49Leave Program
- Paid Sick Leave
- Employees may use sick leave when they are
unable to report to work due to personal illness,
accident or exposure to contagious disease,
and/or for medical appointments when such
appointments cannot be scheduled after working
hours. Sick leave may also be used to attend to
members of the immediate family who are ill
and/or in instances of death in the employee's
immediate family. - 12 Month Employees All staff and administrative
employees earn 15 Sick days per year. New staff
and administrative employees earn 1.25 days per
month in the calendar year that they are hired.
Classified employees earn 1 day a month during
the first year of employment. -

- 12 Month Faculty Employees 15.0 Sick days per
year. New faculty employees earn 1.25 days per
month in the calendar year that they are hired. - 10 Month Employees All staff and administrative

56Energy Holidays
- Holidays which appear with an asterisk () are
days on which classes are recessed and offices
are closed. All administrative, academic and
operations departments will be open for service
during the remaining holidays. The holidays in
which The College remains open are called Energy
Holidays. - Employees who work on Energy Holidays earn Energy
Leave time at a rate of 1.5 times the hours
worked. The Energy Leave time is then used at
designated times during the summer and during the
winter break.

Administrative Employees and Faculty employees
work a 35 hour week at 7 hours per day. The
standard work schedule is 830 am to 430 pm with
a 1 hour lunch. - Full time CWA staff Employees work a 35 hour week
at 7 hours per day. The standard work schedule is
830 am to 430 pm with a 1 hour lunch. - Full time IFPTE employees work a 40 hour week at
8 hours per day. The standard work schedule is
800 am to 430 pm with a half-hour lunch.
58Work Schedules and Leave Accruals for Part Time
Employees
- Part time employees work a proportionate full
time equivalent (FTE) schedule. For example, a
part time employee who works an FTE of .50 works
17.50 hours per week. The daily schedule is
determined by the supervisor. - Part time employees earn the applicable sick,
vacation, and administrative time at the prorated
rate according to their FTE. For example, a 12
month part time classified employee who works at
FTE .50 will earn 7.5 days of sick leave per year
(starting the year after he/she is hired) or
52.50 hours per year and 6 days of vacation per
year or 42 hours of vacation.
59Recording Leave via Monthly Time Reports
- At the end of each month, all full time and part
time employees will receive a monthly time report
which will be scanned on the 15th of the next
month. The administrative assistant in each
department distributes and collects the time
sheets. Complete your monthly time report as
follows - 1. Record the number of hours of leave used on
the appropriate date of the month. Leave the
columns blank if you did not use any leave. - 2. Total the hours of leave used at the bottom
of the time sheet in the sick leave column,
vacation leave column, administrative leave
column etc. Be sure to double check the addition
and bubble in the information in the correct
number columns. Please note that there are 2
decimal points, i.e., 4 should be 4.00. - 3. The employee and supervisor will then sign
and return the time sheet to the Office of Human
Resources. - Please note that detailed instruction on how to
complete the monthly timesheets is available as a
power-point presentation on the Human Resources
web site. -

60Leave Payout upon Termination and Retirement
- When an administrative AFT employee, non-unit,
or classified employee terminates employment with
The College, the employee will receive a payout
for earned but unused vacation and energy leave.
Sick leave will not be paid out. - When an administrative AFT employee, non-unit or
classified employee officially retires from The
College, the employee will receive a payout for
one-half the value of his/her sick accrued leave
up to a maximum of 15,000. - Faculty and Grant employees do not receive a
payout for any earned but unused leave.
61Family Leave Program
- Employees requiring leave because of their own
illness, the birth of a child, or to care for a
seriously ill family member may be covered under
the Federal Family and Medical Leave Act (FMLA)
and/or the New Jersey Family Leave Act (NJFLA). - Employees not eligible for FMLA or NJFLA will be
granted leave in accordance with any legal
requirements governing the type of leave, in
accordance with The Colleges established leave
practices and with consideration for current
business needs. - Employees planning for medical or family leave
should contact the Benefits Administrator in the
Office of Human Resources at (609)771-3306.
62Leave of Absence for Eligible Employees
- Employees injured while in the performance of
their job may be entitled workers compensation
leave benefits. - Eligible employees may apply for voluntary
furlough or paid sabbatical leaves. - Employees in need of leave beyond State and
Federal entitlements should contact the office of
Human Resources for additional information. Such
leaves are granted at the discretion of the
College based upon operational need. -
63Workers Compensation
- Employees who incur work related injuries or
illnesses are eligible to receive medical
treatment through The Colleges Worker's
Compensation Program. When an employee sustains
a work related injury, he/she must report the
injury to a supervisor on the day that the injury
happened. For serious injuries, employees should
call 911 to receive emergency care. - For non-emergency injuries, the employee should
report to the Benefits Administrator and complete
The First Report of Accident. The employee will
then be sent to Professional HealthCare Services
for a medical evaluation and treatment.
64Paid Sabbatical Leave of Absence
- The sabbatical leave is an investment for the
purpose of aiding the qualified recipient to
become more productive and useful to The College.
Sabbatical leave with pay may be granted per the
stipulations in the current AFT union contract. - While an employee is on paid sabbatical, The
College will provide full health insurance
benefits as if the employee were actively
working.
65How to Apply for a Leave of Absence
- When an employee has a need to take a Medical,
Family, or Personal Leave of Absence, contact the
Benefits Administrator in the Office of Human
Resources at (609)771-3306 to discuss your
rights, responsibilities and the impact of leaves
on your employee benefits. - Employees who wish to apply for Sabbatical Leave
should contact the Chair, Dean, or supervisor in
their department.
66NJ State Disability and NJ State Family Leave
Insurance
- Temporary Disability Benefits
- For employees who are temporarily disabled due
to illness or non-work related injury and who
have exhausted all earned sick leave. Benefits
equal two-thirds of average weekly wages up to
559 for a maximum of 26 weeks. (2011 Rate) - Family Leave Insurance Benefits
- For employees who are temporarily unable to work
because of a need to care for a sick family
member or to bond with a newborn baby. Benefits
equal two-thirds of average weekly wages up to
559 for a maximum of 6 weeks. (2011 Rate)
