What happened MTC - CUT TO FINGER REQUIRES
STITCHES - While clearing the boundary line for a
new lease, the chainsaw stopped due to a piece of
wood catching in the chain guard. The worker
stopped, turned off the saw and inspected it.
The worker saw where it was jammed and he pulled
on the piece of wood. When the piece of wood came
free, the workers hand contacted the chain. The
chain cut through the workers glove. The worker
received a cut to his finger which required 2
stitches to close.
- Preliminary Learning Points
- Established procedures not followed. Worker
failed to follow proper procedure for cleaning
debris from chainsaw. Chain bar should have been
loosened prior to dislodging wedge of wood. - Inappropriate risk assessment. Failure to use
alternative tool to remove wedge of wood.
Personnel should never put extremities on or near
sharp objects. - Incident reporting procedure was not followed.
Notification from the field needs to occur within
1 hour of the incident to ensure effective case
management can occur. - Perceived urgency in completing task due
to pending weather.
