    community SLT intervention

  • Patients within Edinburgh City discharged from
    Edinburgh hospitals
  • Good continuity linking with the community SLT in
    Edinburgh
  • Some opportunity for intensive treatment

  • Reconsider the MCN funded post
  • Achieve a more equitable service Lothian wide
  • But
  • Preserve the positive aspects of this post
  • Remember the initial purpose of the post
  • Integrate with 4 separately managed community SLT
    services working across 4 CHPs
  • Maintain the effectiveness of the post avoiding
    over dilution

  • Change the referral criteria to target those
    patients most likely to benefit from the rapid
    response/ increased intensity service

  • Amount intervention received / prognosis
  • Expand the response time target to 5 days
