• Current post set up in 2005
• Set up to bridge the gap between hospital and
  community SLT intervention
• The post has achieved
• Rapid response ( 3 day response target)
• Patients within Edinburgh City discharged from
  Edinburgh hospitals
• Good continuity linking with the community SLT in
  Edinburgh
• Some opportunity for intensive treatment

Objective

• Reconsider the MCN funded post
• Achieve a more equitable service Lothian wide
• But
• Preserve the positive aspects of this post
• Remember the initial purpose of the post
• Integrate with 4 separately managed community SLT
  services working across 4 CHPs
• Maintain the effectiveness of the post avoiding
  over dilution

• duration.
• Edinburgh City has a community SLT service
• East/ Mid SLT service cover hospital sites and
  community
• West Lothian has
• CRABIS (if require 2 or more therapies)
• Hospital out patient service but NO community
  service

• Our objective
• A more equitable, time limited, rapid response
  Speech and Language Therapy Service for priority
  patients being discharged home from hospitals
  across Lothian

Proposal

• Remove the geographical boundary
• Change the referral criteria to target those
  patients most likely to benefit from the rapid
  response/ increased intensity service
• Functional impact
• Context / environment
• Anxiety / insight
• Amount intervention received / prognosis
• Expand the response time target to 5 days
