Title: Mountain View Community Hospital

Mountain View Community Hospital

2
Project Description
  • Mountain View Community Hospital has on out dated
system in place and in order to manage better
they need to consider newer technology for
    Mountain View.
  • They have put in place a special study team along
with a consultant, that will create a system
which would integrate data from heath plans,
physicians, and hospital system, while providing
    real-time information.

3
Current Structure
4
Integrate
  • Patient accounting
  • Care management
  • Insurance systems

5
Potential client/serverarchitecture
6
Options
  • Acquire existing client server application
packages and adapt them to MVCHs needs.
  • Custom development.
  • Layer the client/server applications on top of
    existing systems.
  • Wait for technology improvements.

7
Question 1
  • Why are some of the possibilities for processing
and providing information that could arise if
MVCH were to move to a client/server environment?

8
Answer
  • In building its own custom client/server system,
the short-term costs may be higher. This would
include acquiring advanced tools, the expenses of
training users and the set up time. Since it is
not possible to know at the time when the
decision is made to adopt, the final product may
not look like the expected outcome. With the
existing hardware and software this may be the
best scenario since it offers better
    interoperability.

9
Project Question 2
  • What are some of the possibilities for processing
and providing information that could arise if
Mountain View were to move to a client/server
    environment?

10
Question 2 Answer
  • The system will integrate all data data from
health plans, physician, and hospital systems so
that accurate real time information will be
available. all this implies that the scope of
the system under consideration is very broad.
  • The general system requirements include
availability of real time and accurate
information. Also, an attempt would be made to
    integrate Mountain Views present accounting,
    care management and insurance systems.

11
Question 2 Answer Cont.
  • Our first alternative is the consideration of a
files server system, where all data manipulation
would be used at the clients Personal Computers.
Since the client workstation does most of the
work, each client must be powerful to provide
good response time. The DBMS copy in each
workstation must mange the shared database
    integrity.
  • With the database server architecture, the only
database server requires processing enough power
to handle the database and the database is stored
on the server, not on the clients. The database
server can be tuned to optimized database
    processing performance.

12
Question 2 Answer
  • Since less data is sent across the LAN, the
communication load is reduce. User authorization,
integrity checking, and update processing are all
performed in one location, on the database
    server.
  • Data integrity can be improved as multiple
applications access the same stored procedure.
However, writing stored producers take s more
time than using Visual Basic or powerbulider to
create an application. This reduces their
portability , and may make it difficult to change
    DBMS.

13
Question 2 Answer
  • Parallel processing, however could help Mountain
View to remain centralized on an application or
    database server.
  • If one of the requirements is to have available
    accurate real-time information, the multi-tier
model would make sense. It is suitable for
environments of more than 100 concurrent users.

14
Project Exercise 1
  • In exercise one it wants you to outline the pros
and cons of the four approaches to moving to a
    client/server environment
  • 4 environments are
  • Customizing off the shelf systems.
  • Custom system development
  • Surrounding and layering
  • Doing Nothing for a while

15
Customizing off the shelf systems
  • Pros
  • The Product can be seen before implementation.
  • Beta testing available
  • Quicker implementation. Leading to improved
    productivity
  • Cost estimates are closer to the actual expense.

16
Customizing off the shelf systems
  • Cons
  • Competitors can purchase the same product, and
this will neutralize the expected competitive
    advantage
  • Interoperability with existing hardware and
    software needs to be considered.

17
Custom System Development
  • Tailored to meet all the business specific needs
  • Competitive advantage
  • Needs a project team
  • Additional hidden cost may occur
  • Hard to estimate the actually deadline
  • Better interoperability with the existing
    hardware and software

18
Surrounding and Layering
  • Possible to further utilize the existing
    mainframe system.

19
Project Exercise 2
  • Using the list developed in the first project
exercise, indicate which solution MVCH should
try. Also, indicate what additional information
you would like to have to help you with your
    recommendation.

20
Project Exercise 2 Answer
  • The recommendations may be based on the points
    suggested in the question above.

21
Project Exercise 3
  • Try to get some material about the off the shelf
client/server hospital systems, such as Surginet,
Aviion, and Cycare. Investigate the capabilities
of each system, the number of installations, the
client operating system used, and so on.
Determine which of the systems you have found and
learned about might be of interest of Mountain
    View.

22
Project Exercise 3 Answer
  • Surginet
  • The surgery department is a major cost and
revenue generating center. Management of all of
its resources and the monitoring of utilization
practices provides a significant opportunity to
contribute positively to the financial management
and quality assurance aspects of a healthcare
    organizations.

23
Continued
  • Aviion
  • provides integration and professional services
to over 3500 hospitals worldwide. It provides
customized services that are unique with each
hospital, and has a strategic alliance with
Microsofts healthcare industry and the Intel
    Corporation.

24
Continued
  • Cycare
  • Cycare systems provides information systems,
related support services and electronic data
interchange services to the healthcare industry.

25
Joes Question
  • List the four advantages and describe any 2 of
the four approaches that MVCH has for moving a
    client/server environment.

26
The Four Approaches
  • Customizing off the shelf system
  • Custom system development
  • Surrounding and Layering
  • Doing Nothing for a while

27
Jamess Question
  • There are 3 systems that MVCH might use.
