3
FREQUENCY
  • SORE THROAT, STREP THROAT, COMMONN COLD, FLU,
  • 15-30 Children Adult
  • 4-6 days
  • COMMON SYMPTOMS
  • - Whitish deposition around the throat,
  • - Throat pain, tiredness, cough,
  • - Loss of appetite, difficulty in swallowing,
  • - Thick drainage from the nose, nasal
    stiffness,
  • - Headache, fever, sneezing,

4
MAIN CAUSE(S)
  • Initial viral infection
  • (Adenovirus, herpangina, coxsackie, ..)
  • Weakening of immune system
  • Bacterial growth around the throat
  • (Streptococcus but occasionally mycoplasma,
    Neisseria, ..)

5
NORMAL INFECTED THROAT
6
EXAMPLE OF ECONOMIC LOSSES IN THE USA
  • 75 to 100 million physician are visited annually,
  • Physician cost above 7.7 billion per year.
  • Americans spend 2.9 billion on over-the-counter
    drugs,
  • And 400 million on prescription medicines for
    symptomatic relief,
  • Nearly 36 patients who saw a doctor received an
    antibiotic prescription,
  • Extra costs 1.1 billion annually for estimated
    41 million antibiotic prescriptions,
  • An estimated 180 million school days and 126
million workdays for parents are lost per year,
  • When added to the 150 million workdays missed by
employees suffering from a cold, the total
economic impact of cold-related work loss
    exceeds
  • 20 billion.

7
PHYSIO-PATHOLOGY
  • Virus infection spreads through direct or
indirect contact, infects throat cells, and
replication cycle begins within 8 to 12 hours,
  • Virus attack a few cells, create small lesions,
bacteria attach on the lesions start
    multiplying,
  • Newly liberated virus particles attack new cells,
bacteria continue multiplying and a whitish layer
appears on the throat. This causes pain,
    inflammation, cough formation which gets
    thicken,
  • Natural defence mechanisms neutralise virus and
bacteria but it takes a week to recover.

8
COMMONLY AVAILABLE TREATMENTS (1)
  • Topical antibiotics (ex. Locabiotal, Broncorinol,
    ..)
  • Antibiotics applied topically help to reduce
bacterial load around the throat which takes 2-3
    days,
  • 2. Camphor containing inhalations (ex. Vicks)
  • Decongestion and slight antiseptic effect.
  • 3. Pseudo-ephedrine paracetamol ..(ex.
Actifed, Sudafed) for symptomatic treatment of
headache, fever, nasal congestion alone or in
association with an antihistaminic (ex. Humex),
  • 4. Anti-inflammatory drugs alone or in
association with antipyretic drugs as a
    symptomatic treatment (ex. Rhinadvil)

9
COMMONLY AVAILABLE TREATMENTS (2)
  • 5. Sinus drainage with physiological saline to
    open the respiratory tract,
  • 6. Salt water gargles To create an osmotically
active gradient over the throat so as to
stimulate outward exudation of liquid and
    bacterial detachment,
  • 7. Topical analgesics and anaesthetics (ex.
    Lozenges to reduce pain),
  • 8. Different plant preparations,

Inconveniences of existing drugs
  • Mostly symptomatic treatment,
  • No anti-viral effects,
  • Antibiotics No instant effect,
  • Antihistaminic Dizziness, dry mouth, dry eyes
    and nose, constipation, confusion, visual
    troubles, excitation,.
  • Anti-inflammatory drugs Only reduce inflammation.

13
WHICH IS THE BEST AVAILABLE TREATMENT?
  • LUKE-WARM SALT WATER GARGLES!
  • Relief Millions of persons in poor countries use
salt water gargles often get relief within 1
    day.
  • Mode of action Salt molecules creates an
osmotically active film over the throat surface.
This causes outward exudation of liquid from the
tissues covered with salt to equilibrate osmotic
pressure between inner outer surfaces. This
leads to instant detachment elimination of
    micro-organisms.
  • Why salt water gargles are not used? Very few
persons know this activity, tedious to follow 3-4
times a day, may cause vomiting.
  • Why no pharmaceutical company launched a salt
gargle product? Salt is a natural product which
    cant be patented!
  • What is the disadvantage Bad taste, gargling
procedure, time required (4-5 min each), no
hydrating effect, no fluidifying effect, no
    anti-viral effect..

14
QUALITIES OF AN IDEAL TREATMENT?
  • AN IDEAL TREATMENT SHOULD
  • Have instant effects (1-2h),
  • Detach bacterial layer from the throat surface,
  • Exert antibacterial effect,
  • Neutralise free virus particles to stop new cell
    infection,
  • Remove detached micro-organisms from the throat
    vicinity,
  • Hydrate and soothe throat,
  • Drain cough,
  • Not enter into the throat tissue,
  • Not be toxic when swallowed,
  • Be natural, colourless, odourless, with agreeable
    taste.

15
VB-TH4
  • THIS IS A 15-ml SPRAY CONTAINING FOUR BASIC
INGREDIENTS PRESENTED AS A MEDICAL DEVICE.
  • VB-GYL A patented natural hypertonic solution.
  • VB-HY A natural food ingredient with strong
    antiseptic and anti-bacterial properties.
  • VB-PCD Specific tannins isolated from a plant
having strong affinity for viral proteins.
  • VB-TEA A specific plant tannin with strong
    topical anti-inflammatory properties.

16
1. VB-GYL
  • This is an osmotically active, natural, dietary
plant by-product patented throughout the world by
    VITROBIO for its anti-microbial healing
properties. This product act by creating an
osmotically active film over the throat surface
leading to outward flow of liquid from inside
towards outside the cells. This cause instant
detachment of all contaminants from the cell
    surface.
  • Used as an excipient (diluent) in many
    pharmaceutical, cosmetic, and dietary
    preparations.
  • Cant enter into the cells and acts through its
    physical properties.

17
2. Anti-microbial effect - VB-HY
  • This is a natural food ingredient, osmotically
active, known as a strong antiseptic as an
    excellent antibiotic.
  • Stimulates liquid secreation from the throat
tissues acting as a natural anti-microbial
agent and as an expectorant to fluidify the nose,
  • Used as food,
  • Cant enter into the cells and acts through its
    physical properties.
  • An excellent healing agent.

18
3. Anti-viral effect- VB-PCD
  • A procyanidine (tannin), large complex molecules
(mol. wt. between 500-20000) present in the
    plants, containing sufficient hydroxyl carboxyl
groups to form strong complexes with proteins
(viral protein) and other macromolecules so as to
    precipitate these molecules.
  • Bounded virus particles cant infect new cells.

19
Influenza virus does not have the processing
protease for viral membrane fusion glycoprotein.
Therefore, to enter into the cell, virus uses
host cell proteases that proteolytically activate
virus-cell fusion.
  • Protease
  • Virus

20
MODE OF VIRAL CELL INFECTION
  • Virus alone cant enter into the cell
  • Virus associates with proteases
  • 3. Host cell protease activate virus cell fusion
  • 4. Intracellular virus replication causes cell
death, formation of lesion subsequent bacterial
    infection.

21
EXISTING OR POSSIBLE ANTIVIRAL DRUGS
  • Supply nuriminadase inhibitors but viral
resistance is a common phenomenon (ex.
    Oseltamivir, Zanamivir, peramivir),
  • Up-regulate the production of endogenous protease
    inhibitory compounds,
  • Increase mucosal immunity (ex. With Ambroxol)
  • Increase IgA level (ex. With the macrolide
    antibiotic Clarithromycin),
  • Stop viral penetration or replication into the
    cells,
  • Give additional topical protease inhibitors.

22
ANTI-VIRAL MODE OF ACTION OF VB-TH4
  • Due to strong affinity for viral glyco-proteins,
VB-TH4 procyanidin I bind neutralise virus
    perticles.
  • VB-Th4 procyanidins
  • Virus
  • Procyanidin-virus binding

23
ANTI-PROTEASE MODE OF ACTION OF VB-TH4
  • VB-TH4 Procyanidin II binds to proteases
    (Trypsin, elastase, ..). Stopping virus-cell
    fusion further cellular infection.
  • VB-Th4 procyanidins
  • Proteases
  • Procyanidin-Protease binding

24
4. Anti-inflammatory effect - VB-TEA
  • A naturally occurring tannin with a strong
    proven anti-inflammatory properties.

25
WHY PLANT TANNINS?
  • Vitrobio isolated, purified tested hundreds of
plant tannins for specific properties. Some
tannins binds to specific viruses or exert
    anti-inflammatory effects. Tannins are big
molecules cant enter the cells. Due to their
strong binding properties with other molecules,
they are also used to enhance product viscosity.
Due to their physical mode of action, tannins can
be used for external application as a medical
    device.

26
