7What is transition planning?
- Transition (noun) 1. movement, passage, or change
from one position, state, stage, subject,
concept, etc., to another 2. change the
transition from adolescence to adulthood. - Transition Planning - Making and carrying out a
very intentional plan for the change to your
adult life in the areas of living, working and
posthigh school learning.
8What is Transition Planning in High School?
- Outcome-oriented process focusing on what is
needed for adult life functioning. - Includes a coordinated set of activities,
provided to the student by the school and
sometimes other agencies, to promote a successful
transition from high school to postsecondary
education or employment, and independent living.

parents and agencies as your partners. You know
what you need to meet your career / life goals

match of your skills and life options in the
areas of earning, learning and living.
