What is transition planning?
  • Transition (noun) 1. movement, passage, or change
    from one position, state, stage, subject,
    concept, etc., to another 2. change the
    transition from adolescence to adulthood.
  • Transition Planning - Making and carrying out a
    very intentional plan for the change to your
    adult life in the areas of living, working and
    posthigh school learning.

What is Transition Planning in High School?
  • Outcome-oriented process focusing on what is
    needed for adult life functioning.
  • Includes a coordinated set of activities,
    provided to the student by the school and
    sometimes other agencies, to promote a successful
    transition from high school to postsecondary
    education or employment, and independent living.

An Ideal High School Outcome You eventually run
your own IEP Meetings with your teachers,
parents and agencies as your partners. You know
what you need to meet your career / life goals
and how to get what you need to be successful.

    learning and independent living

  • Find out how you can build skills and make a good
    match of your skills and life options in the
    areas of earning, learning and living.

    parents and teachers to assess your skills,
    strengths and needs.
  • Make a plan to build on your strengths and work
    on your needs.
