Safety Culture
  • Organizational Culture
  • the shared basic assumptions that are developed
in an organization as it learns and copes with
    problems.
  • the basic assumptions that have worked well
enough to be considered valid are taught to new
members of the organization as the correct way to
    perceive, think, act, and feel.
  • Safety Culture
  • An organizations values and behaviors modeled
by its leaders and internalized by its members
that serve to make nuclear safety the overriding
    priority.
  • INPOs Principles for a Strong Nuclear Safety
    Culture

3
IST Culture IssuesA Team Effort?
  • Who owns the IST Issues?
  • IST Engineer?
  • Component Engineer?
  • System Engineer?
  • Maintenance Engineering or even from a Broke/Fix
    standpoint, Maintenance?
  • Each site will have their own unique issues, or
    culture.
  • Work in progress, constantly evolving as we
    address issues
  • Detailed Program Documents will help improve your
    IST Culture
  • IST Program Bases Document (LAN) to show what
    is/what is not/and why.
  • IST Database (LAN) with adequate depth of
    content.
  • IST Implementing Procedures Generic, Pump,
    Valves.
  • Visible Interface with other key groups on site.

4
IST Culture Some Seabrook issues where we are
working to get ahead of the issue
  • Instruments used for IST out of tolerance
Adverse trend? What is the interval? First time
    event?
  • Stroke Time What is the trend? How to know
when it is adverse what rules are applied?
  • IST Pump - Close to ALERT value, or fails
surveillance test What was the trend? What/who
    is tracking the issue?
  • Relief Valve fails setpoint test Repeat?
Contingency? Expanded Scope? What other valves
are impacted? What S/N is involved?
  • Relief Valve not tested within required timeframe
How and who is tracking? Change in model?
  • Check Valves How to describe Check Valve
    Condition Monitoring activities Forward,
    reverse, interval, PMT issues?

5
IST Culture IssuesInstrumentation
  • Pumps
  • Recent IC Design calculations for Accuracy
  • IC Maintenance Procedures or PM Tasks scheduled
    in Work Management process
  • Protected steps for key attributes MTE,
    intervals, when to adjust, etc.
  • Known locations for this info.
  • Reviewed by IST Group for acceptability.
  • Adequate Technical Rigor used in these reviews?
Partnering with Pump and System Engineer for
    margin issues? (KPIs)
  • Database feature select by instrument,
historical sheet showing AF/AL results, past ARs
    and corrective actions.
  • OX1456.86 contains all relevant acceptance
criteria info used as the technical bases for
    Operations.

IST Culture IssuesStroke Time Variations
  • What has the historical trend been? Slow and
gradual?, versus sudden step? Margin issue?
  • How determined? At- a-glance, or graphical, or
    statistical analysis?
  • When does troubleshooting begin? Some valves,
like AOVs, require partnering with Program
    Engineer (KPIs)
  • Immediate retest performed?
  • Control Room or local?
  • Database feature graphical presentation
  • OX1456.81 contains all relevant acceptance
criteria information used as our technical
    bases for Operations.

IST Culture IssuesPump Issues
  • Hydraulic Condition
  • what is the bases?
  • Degradation taken in long term view (e.g. stair
step versus basement view), Head curve as
    original test bases? (KPIs)
  • Scatter issues used to improve test methodology?
  • Mechanical Condition
  • Smooth running pumps and use of analysis being
    identified as margin issue?
  • Design issues identified with corrective actions?
  • Operations interface ON LINE versus Outage
  • Baseline procedure protocols
  • Shop head Curve test with vibrations as practical
  • In situ Head Curve performed with IST (patience)
  • CBM taking vibrations and performing analysis

IST Culture IssuesRelief Valve Issues
  • RV Issues
  • Some Design changes to replace obsolete valve
    designs
  • Change in trending
  • Standardization of tail pipe to permit
    interchangeability
  • Outage Planning for failures train rolls,
    additional testing.
  • Not much expanded acceptance criteria (PZRs,
    MSSVs for AF testing)
  • Hot/Hot Testing
  • ES1850.011 Procedure, EX1804.044 for IST Valves
  • Tracking of AL post refueling Outage??
  • In house AR
  • PZRs shipped

IST Culture IssuesCheck Valve Condition
Monitoring Issues
  • Two separate databases
  • One on LAN and one in Group Drive
  • Needed increased visibility
  • Database Demonstration
  • Analysis bases
  • Forward Activity by activity/interval
  • Reverse Activity by activity/interval
  • Technical Rigor
  • Two person review next outage focus.
  • Instrumentation issues
  • instrument accuracy

  • Post Maintenance Testing issues
  • Planner Interface
  • Component Engineer interface
  • Outage Planning correct sequencing
  • Performance Monitoring
  • PWR PIV testing performance based
  • Trending for next outage issues
  • Transfer to OPS for SI Test Header Operation
  • Not just getting to SAT
  • Sequencing of the testing test second outs
before first outs get seated by RCS Pressure.

39
IST Culture IssuesTrending vs. Data Entry
  • Data Entry does not equal trending
  • Technical Specification compliance verification
of the activity and that it meet acceptance
    criteria
  • Performance Monitoring review - multi layered
  • Identify adverse trend
  • Focus on next outage activities
  • Adjust knowledge of component performance
  • Interface with System, Component, or other
