  • 57 yo male develops sudden onset headache and
    left sided weakness
  • Family calls 911 (112, 115, etc)
  • EMS transport to hospital
  • Symptoms progress to full hemiplegia
  • Initial VS 210 / 120 mmHg, HR 110, R 24

Pathophysiology
  • Initial hemorrhage into tissues causes
  • Cytotoxic and vasogenic edema formation
  • Mediators MMP-9, inflammatory response, blood
    degradation products
  • Elevated intracranial pressure due to
  • Hematoma mass effect
  • Perihematomal edema
  • Intraventricular extension and hydrocephalus
  • Decreased regional perfusion and herniation
