• IPs finger got trapped between the descending
  assemblies of the iron roughneck causing crush
  injury.
• CAUSES
• Lack of due care and attention
• Executing the job as opposed to supervising the
  job
• LEARNINGS
• Whilst Supervising do not step in to assist in
  the job
• Always watch out for pinch points
