Purpose

• To create a master list of clinics offering
  primary care to children 0-5
• To assess the number of clinics offering
  immunization services to children 0-5
• Follow-up to Registry Workgroup 2 question What
  electronic medical record/billing systems are
  used?

Clinic Survey Questionnaire

• 11 questions on immunization practices
• Questionnaire mailed to every clinic identified
  from the physician-linkage database
• Survey mailed out September 1st
