What happened PLD-FRAME CRACKS ON WORKOVER RIG.
While lowering the derrick, unit frame cracked,
allowing the derrick to drop 3 feet to the
cradle. During the incident the operator's elbow
was struck by the brake handle causing no
apparent injury.

• Learning points
• Observe barrier zones, especially when
  raising/lowering derrick.
• Remind crews of the unexpected happening.
• Always cover escape routes during job planning.
  The operator avoided injury by utilizing the
  stairs to egress vs. jumping off the truck.
