• Daily electronic home monitoring of weight, BP,
  heart rate, and blood oxygen levels.
• Pre-programmed questions regarding symptoms.
• Education provided electronically.

Target Group

• 35 clients
• Enrolled in DTHR Heart Function Clinic
• Minimum 6 months in program
• Clinically unstable chronic heart failure
• NY Level II - IV
