winch up a rubbish skip.Simple! What could go
wrong ???

- Fahad stands on the wrong spot behind the truck
signalling the driver. - The driver does not see him
- The driver reverses
- Fahad is crushed to death

- It was Ramadhan
- It was the end of the day, the last load
- It was getting late
- Fahad was very young and new on the job (4 weeks)
- The crew and the truck operated under a
subcontract - Do these issues add to the risk of a simple
operation? - How do you control/manage that increased risk?
7- REVERSING RULES for DRIVER
- Check the surrounding and plan the manoeuvre with
the helper - Do not move unless you can see the helper
- Act as per agreed hand signals from the helper
- Use your mirrors
- STOP the vehicle if in doubt
- Do NOT rush.
- REVERSING RULES for HELPER
- Must get down from the vehicle
- Must stand away from the vehicle
- Must always have eye contact with the driver
- Must use agreed hand signals
- Shall NEVER stand behind the moving vehicle
- STOP the vehicle if in doubt
- Fahad would still be alive if these rules had
been followed
