  • Lays head on desk during instruction

  • Write IN DETAIL on the IEP what steps will be
    taken if the behavior occurs
  • Plan should be written in a positive way
  • Write what behaviors you want to see the student
    exhibit.
  • Example Tom will be in his seat when the bell
    rings not Tom will not be tardy for class
  • Positive feedback should be given when correct
    behaviors are being exhibited

Destruction of Records
  • Notify parents of right to review records
  • Parents request of Amending Records
  • Parent can make a written request
  • Clearly Identify the part of the record to be
    changed
  • Reason why it should be changed
  • If school disagrees, notify parent reason for
    decision and advise the parent their right to a
    hearing
  • Procedures after students exit
  • After Student exits the educational program, keep
    records 5 to 7 years.
  • Must notify parents/students of proposed
    destruction (Pg 11)
