  and post it together with the following

• Photocopy I/C of applicant and beneficiary
• Prove of relation with beneficiary (Birth
  certificate, marriage certificate or etc)

Requirements To Remain Active

• Possess a minimum balance amount of 10 MyPoints
  in MyPoints Account at any time.
• Transact a minimum purchase of RM30.00 worth of
  prepaid cards or MyMode products each month
• You will lose the benefit of making compensation
  claims if you are unable to fulfill the
  conditions above on a monthly basis.

• What should your beneficiary do in filing the
  claim?
• 1. Lodge a report to Mobile e-Transaction Sdn Bhd
  (Attention to MCCP Unit) within 7 working days
  from the date of demise and append the following
  items
• Original copy of police report
• Death certificate
• I.C. or birth certificate (if below 12 years of
  age) of beneficiary
• Call our AIC to alert the person in charge and to
  ensure your document has been received.
• Mobile e-Transaction Sdn Bhd will contact you for
  a cheque bearing the compensated amount on the
  15th of the month following the demise of the
  participant.
• Note
• MCCP will only consider compensation claims for
  deaths due to accident or other natural causes
  leading to the death of the MyMode Agent e.g.
  illness. MCCP will not compensate deaths due to
  suicidal attempts or intentional injury that
  leads to death.
• Compensation claims can only be made by the
  beneficiary if the benefactor has been a
  participant of MCCP for 3 months or more.
• The beneficiary strictly for immediate family
  member only (either husband, wife, father,
  mother, son or daughter).
