What Is the Interactive Survey System?

• Online, interactive version of the documents
  needed for NCQA accreditation, certification or
  recognition programs
• Two components
• Standards and Guidelines (SGs)
• Survey Tool (ST or Tool)

So What Does It Mean for You?

• ISS Tool released April 15
• Tools sent for each SNP benefit package
• Completed Tools due to NCQA no later than June
  30, 2008
• Survey is final once SNP clicks SUBMIT
• No organization comment
• No conference call with individual organization
• No reconsideration
• No Accreditation status

Dont Wait to Submit

• Structure Process Measures due June 30, 2008
• SNPs do not have to wait until June 30 to submit
  their SP Survey Tool. If you finish early, click
  SUBMIT early.
• Heavy volume at the end of June may cause delays.
  The sooner you get it in, the smoother it will be.

Survey Process

• Organization submits completed Survey Tool with
  attached documents
• NCQA survey team assesses compliance with all
  Structure Process measure requirements
• Final scores analyzed for inclusion in report to
  CMS
• NCQA will not publicly report SNP data

Developing Your Documentation System

• Build an electronic system for all documentation
  to be linked to Survey Tool
• Not required to keep documents in a central
  file.but this is a common practice
• Use any organizing principle you wish
• May pull from different areas may help in
  version control

ST Documentation Library

• Central reservoir of all reference material for
  compliance
• Sort by document, standard, file path and date
• Load into the library and then link with specific
  elements

Document Uploading

• Start early
• Send documents in bunches
• Copy your documents from your network to your
  hard drive, and upload from there
• Upload during off-peak hours

Documentation Requirements

• For security and data protection only allow file
  types
• .CSV, .DOC, .GIF, .JPG, .MPP, .PDF, .PPT, .RTF,
  .TXT, .VSD, .XLS
• Convert others to .GIF, .JPG via scanner or to
  .PDF using Adobe Acrobat
• Do not send Zip files to NCQA

Documentation - Scanning

• To minimize file size and retain readability
• Save scanned documents in .GIF, .TIF, .JPG format
• Color images 80 dpi and 8 bit (256) color
  settings
• Text pages 80 dpi and 2 bit (black and white or
  line art settings)
• Black/white image 80 dpi and 8 bit (256 color)
  settings or 80 dpi and 8 bit scale (256 shades)
  settings

Documentation Tips

• Highlight key sentences
• Focus on only what element requires
• Limit size and number
• Use only pertinent pages with cover page that
  provides the context
• Unless element requires multiple documents, one
  is sufficient
• Use hyperlinks in documents if you wish

What Should I Put in the Support Text Box?

• Use this area to communicate with your survey
  team
• Utilize page paragraph number field
• Brief summary of information focus on what the
  element requires
• If element has multiple factors, consider using
  this field to map out information

How to use the Comment Box

• Comment Box is different from the Support Text
  Box
• Requires SNP to include additional information
  related to a specific element or factor
• SNP 1 Element A Comment Box
• NCQA is looking for information regarding the
  eligibility criteria used by plans for case
  management and the of members enrolled in case
  management
• SNP 1 Element B Comment Box
• NCQA is looking for other referral sources for CM
