Employees entered sewer w/o completing permit and
were completing excavation in the dark. Lengthy
PRCS and safety meeting discussion followed.
Public atop Metal Matic, Inc roof - instructed to
get off and they complied.
Employees allowing civilians and unauthorized
people in work zone.
Giving tour wearing sandals.
Installing fences without hearing protection.
Driving Rangers up and down the hill and through
trees.
Driving through red zone to access check point.

Incidents
Truck pulled out over chocks and car fell from
flatbed. Area was clear/no injuries. Procedures
changed to secure car before exiting red zone.
5 city employees were deploying a nonmotorized
boat into drainage canal without PFDs. Contact
with MPD made and employees donned PFDs.
The Sheriff was found without PPE when vehicles
were being moved overhead. He stated that he was
with a Congressman and didn't need any. Bolander
ceased all activities until the visitors left the
site.
