Plan Structure
  • PHBP covers eligible non-union freelance
    production employees.
  • Those eligible include all production employees
    not covered by a Union plan. (The job titles
    include Producer, Line Producer, Production
    Supervisor, Assistant Production Supervisor, and
    Production Assistant).

Employer Responsibilities

    administer the Producers Health Benefit Plan.
  • ASO has a great deal of experience administering
    health funds in the entertainment industry. Their
    clients include organizations currently providing
    benefits to some union employees of AICP members.

Authorization Forms
  • Authorization Forms allow the Payroll Companies
    to release data to ASO to determine eligibility
    for the Plan.
  • Forms must be remitted by September 30, 2007 in
    order to determine eligibility for the Plan,
    which begins February 1, 2008.
  • Once 1,000 freelance employees are deemed
    eligible, UHC will be able to design the Plan
    benefits.

Employee Participation
  • Qualifying employees include those Freelancers
    working in a production capacity with the job
    titles of Producer or Line Producer, Production
    Supervisor, Assistant Production Supervisor, and
    Production Assistant.

  • There is no cost for coverage of a qualifying
    employee, except if they elect to cover their
    dependents (including qualified domestic
    partners), which will be billable directly to the
    employee and entirely at their expense.

  • Employees data must be received by Sept. 30 in
    order to determine eligibility for the 1st
    quarter of the 2008 Plan Year.
  • Employees must have worked at least 30 days per
    quarter for a full year to qualify.

  • All days in excess of 30 in a quarter will be
    banked. These banked days may be used to make
    up for short falls of the 30 days per quarter
    needed for qualification. The bank holds up to 90
    days, and all banked days must be used within 3
    quarters of the quarter that they were earned.

  • If a qualified participant does not have enough
    banked to reach 30 days in the quarter, they can
    self-pay the difference, provided they have
    worked at least 20 days in that quarter. This
    option can be used only for one quarter at a
    time. If this occurs in a second continuous
    quarter, the employee may elect COBRA
    continuation coverage.

  • Initial eligibility begins with the fourth
    quarter of 2006 (Oct. Dec.) through the third
    quarter of 2007 (July Sept.). For calculation
    purposes, the banked bridging rules apply
    (e.g. if an employee worked 60 days in the second
    quarter of 2007, and only 20 days in the third
    quarter and worked 30 days in Q4 of 2006 and Q1
    2007 he or she would still qualify for the
    Plan).

  • All days worked will be credited (provided
    contributions are received by the Plan) except
    for when an employee works for an employer not
    headquartered in AZ, CA, NY, Il, IN, Iowa, PA,
    MA, MD, or VA. In a case where a company is
    headquartered in one of the other 40 states, only
    work for that company which takes place outside
    the state where the company is headquartered will
    be credited.

Loss of Eligibility
  • The freelancer will lose eligibility for
    continued participation if the freelancer fails
    to meet the Standard and Bridge eligibility
    requirements.
  • When coverage ends for this reason, the
    freelancer will have an opportunity to continue
    their coverage under the federal law known as
    COBRA. COBRA requires that in certain
    circumstances health plans must offer the ability
    to self-pay for group coverage for a limited
    period of time. There will be more details on
    COBRA in the summary plan description.

Dependent Coverage
  • Eligibility for coverage of Freelance dependents
    generally begins on the same date that your
    coverage starts or, if later, when they first
    become the Freelancers dependents.
  • Coverage will not begin if the Freelancer does
    not provide necessary proof of dependent status.
