Open Enrollment

• Berkcom Benefit Open Enrollment will be held
  until September 14, 2007.
• All eligible employees must complete a benefit
  selection form.
• Unless there is a change of status during the
  benefit year, employee will not be able to change
  his/her selected coverage.
• Benefits effective date is October 1, 2007.
