  • Scan the barcode on your employee ID
  • If you do not have your ID badge you can type in
    your ID

Identify Patient
  • There are 3 ways to identify a patient and access
    their record.
  • Scan the patient wrist band barcode
  • Type in the patient account number
  • Select a patients name from a drop down menu
  • Of these, only scanning the patients wrist band
    barcode does not require a second level patient
    identification.

    another step before you are allowed to do

    anything other than view information.
  • To go any further the patient ID must be verified
    by either scanning the wrist band OR typing the
    account number.

  • The Bridge MedPoint Medication Management System
    receives patient order information from the
    pharmacy order entry system.
  • Ensuring that the pharmacy receives orders as
    quickly as possible will help the orders to
    display on the bedside device in a timely
    fashion.
  • Once the patient wrist band barcode has been
    scanned, one of 2 screens will appear. If there
    are no new orders, the patients medication
    profile will be displayed. If there are new
    orders to be confirmed the Orders screen will
    display (Order Confirmation is covered in
    detail later in this program).

  • There are six tabs to choose from when you select
    the Medications task button. Think of them as
    pages
  • Scheduled medications

Scheduled Medications Tab
  • Scheduled medications are listed according to
    when they are due.
  • Last Given column shows the last time the med
    was administered.
  • LATE medications PINK
  • (and show a clock next to the due time)
  • CURRENTLY due medications YELLOW
  • FUTURE due medications have no highlight

PRN Medications Tab
  • PRN meds are listed in alphabetical order
  • Last Given column shows the last time the med
    was given.
  • When only the time is showing (no date) it means
    the current date otherwise both the date time
    are shown.

  • The IV tab lists orders for continuous IV orders
  • Only IV that infuse continuously show here
  • IVs are listed in alphabetical order
  • Last Given column shows the last time the IV
    was given (generally meaning hung)

    is no need to access the Floorstock or Formulary

When Administering a Med Prior to Pharmacy Order
Entry
  • REMEMBER If the order has not been entered into
    the pharmacy system, chances are the pharmacist
    has not had the opportunity to review the
    medication order. In this situation it is the
    nurse who is responsible for making sure that the
    appropriate checks are done prior to
    administering the medication.
  • If the medication you want to administer has a
    barcode, scan it. There is no need to access the
    floorstock or formulary lists.
  • It should be the RARE occasion that requires the
    nurse to go to the formulary list. Not all
    medications on this list are approved for the
    nurse to administer without pharmacy approval of
    the order.

Scanning Meds to Administer
  • Scan the med
  • The medication will move from the due list to the
    Meds Selected list.
  • If a medication does not have a barcode or the
    barcode can not be read, select the medication
    from the list.

  • Select the Finish button once all selections
    have been made.
  • If multiple selections were made, the last
    scanned item will always be in the first
    position. Click on drop down arrow to view the
    entire list.

    changed or edited.

Unlink Dose
  • Unlink Dose allows the user to document the
    administration of a dose for which there is an
    order but not a dose listed as due.
  • Example The patient is admitted at 10 AM and
    has an order for Lasix 40mg PO BID. The bedside
    device displays the 8 PM dose but not the 8 AM
    dose since the order was entered after the dose
    time had passed.

Unlink Order
  • Unlink Order allows the user to administer a
    newly ordered dose of medication, for which there
    is an existing order in the system, prior to
    pharmacy order entry.
  • Example Give an additional dose of Lasix 40 mg
    PO STAT. This could be a STAT or additional dose
    of a medication for which there is currently
    another order in the system. If the dose or order
    is not the one you wish this administration to be
    charted against select Unlink Order

Warnings and Alerts
  • Warnings or Alerts may be generated with certain
    activities.
  • OK continue on with activity
  • Cancel Stop, return to last screen
  • Unlink start the Unlink sequence (this button
    is not on every warning screen)
  • If a drop down menu to select a reason is
    present select the most appropriate reason for
    the situation at hand. Remember free text
    reasons are not allowed and do not get saved.

  • What is Multiple Order Match?
  • Sometimes patients have 2 orders for the same
    medication. An example, a scheduled order and
    a PRN order for the same medication such as
    Insulin or Potassium
  • When the medication is scanned, MedPoint knows
    that a match exists and presents you the
    opportunity to select which order the dose you
    are giving should be documented against

  • Select the order to associate with make sure
    the small circle on the left is marked

Co-Signature
  • What is Co-Signature why do we have it?
  • Co-signature is a second witness to a drug
    administration.
  • Drugs There are particular drugs, such as
    insulin or chemotherapy that are considered high
    risk might need a co-signature prior to
    administration.
  • Person - Certain people such as nursing students
    may be required to obtain co-signatures before
    administering drugs.
  • Places - Certain units (oncology) might have a
    policy that requires co-signature for all
    medication administrations.

  • When a specific drug requires a co-signature, the
    pharmacy may use a comment to alert you.
  • After scanning the medication barcode, the dose
    detail screen will appear.

  • In any of the situations (drug, person or place)
    that require a co-signature the dose detail
    screen appears and there is a notice of
    required next to the co-signature field.

  • What are Allergy Alerts?
  • Allergy alerts tell the nurse something about the
    allergy status of the patient.
  • Types of Allergy Alerts
  • No allergy information has been received.
  • New allergy information has been received.
  • Possible allergy or cross-sensitivity to the
    medication being given has been detected.

  • This alert is triggered when MedAdmin has not
    received any allergy information from the other
    system. This alert is seen each time the patient
    is selected until allergy information is received
    by MedAdmin (even NKA).

Confirming OrdersThe PROCESS
  • All new orders entered by pharmacy will display
    on the bedside device and the nurse will use
    MedPoint to confirm them.
  • The process of confirming orders creates the
    doses which display on the Medications screen.
  • ALL ORDERS, whether active, discontinued, future,
    on hold, or resumed will need to be confirmed in
    this manner.
  • If there are new orders to be confirmed, the
    Orders screen will display first after scanning
    the patients wristband barcode.

Confirming Orders What does it all mean?
  • Order Information - review this for accuracy
    against the written order in the chart
  • Order Start Time/date indicates the time that
    doses will begin to be created for Active or
    Resumed orders
  • Special Note In the case of a DISCONTINUED
    order this time/date indicates when the doses
    will be removed from the screen. If the D/C
    order has doses in the LATE (pink) status, they
    will NOT be removed, remove or not give these
    doses according to hospital policy.
  • Order Stop Time/date indicates the time that
    was entered for that medication to be D/Cd or to
    expire. Doses will not be created beyond this
    time.

Confirming Orders More what does it all mean?
  • Order Status only one order status is assigned
    to an order. An order can be
  • ? Active ? Hold
  • ? Discontinued ? Resumed
  • ? Future ? Invalid
  • Confirm or Clarify Select one of these 2
    actions for each order.
  • Confirm when you confirm an order, you are
    agreeing with all the order information as well
    as the order start/stop time.
  • Clarify when there is a discrepancy between the
    order on the screen and the written order, you
    put it into clarify state until you have
    contacted pharmacy and the matter is resolved.

  • After you have followed your hospitals order
    confirmation procedure touch the Confirm
    button.
  • The medication will be transcribed to the
    Scheduled, PRN or IV medication list. Check to be
    sure the medication appears on the tab as
    indicated.

  • When an order is Discontinued, check the
    Scheduled Med screen to ensure that no doses are
    left behind.
  • Doses that are left on the Scheduled Med screen
    after the
  • Start Time will need to be REMOVED or NOT GIVEN
  • according to hospital policy.

Confirming OrdersThings to Remember Start Time
  • The Start Time of an order is important. It is
    used to
  • Create doses for NEW or RESUME or Updates to an
    existing order.
  • Eliminate futures doses for DISCONTINUE or HOLD
    orders. Please note - doses that are created up
    until the Start time of the D/C order, will NOT
    BE REMOVED.
  • ALWAYS check the doses on the scheduled meds
    listing to ensure doses are listed as expected.
  • REMEMBER After confirming a D/C or HOLD check
    to see that no doses are left behind. If there
    are, Remove or Not Give those doses according
    to hospital policy.

Clarifying an Order
  • Putting an order into the clarify state
    indicates to others that you are questioning the
    order.
  • To do this simply select the CLARIFY button

  • After selecting the Clarify button a reason
    list appears
  • Select a reason from the drop down list

Remember Select only from the list, no free
text is allowed or saved

  • Note that the clarify order is now highlighted
    blue. The reason selected also appears.
  • IMPORTANT Notify pharmacy immediately of the
    need to clarify the order. MedPoint DOES NOT
    communicate to the Pharmacist or the pharmacy
    system !

Clarifying an Order
  • Once an order has been put into a clarify
    state, it can only be confirmed.
  • If the order is found to be correct, confirm it.
  • If the order is INCORRECT, it will need to be
    D/Cd by the pharmacy.

Invalid Orders
  • INVALID orders appear when MedPoint does not have
    the information to display the order correctly
    (i.e. invalid route, invalid frequency, etc.)
  • MedPoint displays the reason the order is
    Invalid. The reason appears after the
    medication name in parenthesis.
  • Notify pharmacy, they will have to correct the
    invalid component resend the order. Then the
    invalid order will no longer display
  • Invalid orders CAN NOT be confirmed or clarified

Linking Administrations
  • If a medication was administered prior to order
    processing AND an Unlink Order was done,
    MedPoint will give you the chance to Link that
    medication administration with the order during
    the confirmation process
  • Select Confirm

  • An alert that an Unlinked Order administration
    matches the order
  • Select the circle next to the medication name
  • Select OK the administration is then Linked
    to that medication
  • NOTE Linking the dose here DOES NOT remove any
    doses created when the order was confirmed

Linking AdministrationsThings to Remember
  • Link medication administrations to appropriate
    orders. This ensures the documentation on the
    printed version of the eMAR is accurate.
    Administrations linked to an order are documented
    along side that order.
  • Medications that are administered Unlinked or
    prior to order entry will be documented in the
    Other Med category on the eMAR.
  • Please REMEMBER
  • Linking to an order DOES NOT LINK IT to a
    SPECIFIC DOSE.
  • If you are linking a scheduled med to an order
    MAKE SURE that the DOSES that are listed as due
    are accurate.
  • Always check to be sure that any previously
    administered doses (prior to confirming the
    order) are NOT LISTED as needing to be
    administered. Remove any doses that should not
    be administered.

Confirming OrdersA Review
  • Order Confirmation Information Review
  • The initials of the user who confirmed the order
    will display on the printed MARs and other
    reports.
  • Once an order is confirmed it may not be
    unconfirmed
  • Once an order is put into a clarify status it can
    only be confirmed.
  • Only the pharmacy system has the ability to
    change the order.
  • Users may not confirm orders while in the process
    of administering medications. If an Unconfirmed
    Order warning is received the user must confirm
    or delete medications selected in order to access
    the Order Confirmation functionality.
  • Invalid Orders can never be confirmed.
  • Only one order may be confirmed at a time. You
    cannot select multiple orders to confirm in a
    single transaction.
  • All orders should be confirmed in the order in
    which they were received the oldest order is
    at the top of the list. A warning is triggered
    if the user tries to confirms updates out of
    sequence.

  • Start Time reminders
  • All orders display a start time that will
    display.
  • Doses are CREATED based on order start time
  • Doses are ELIMINATED based on start time for D/C
    or Hold orders.
  • If the start time is in the past MedPoint creates
    the doses from the start time and display as late
    or currently due based on dose due time windows
  • The UNLINK DOSE function allows the nurse to
    administer a dose before the first dose without
    stealing a future dose
  • Stop Time reminders
  • Not all orders will have a stop time.
  • If a stop time is entered, dose will not be
    created after that time.
  • Linking a med to an order does just that, it
    links it to the ORDER. It DOES NOT LINK the med
    to a specific DOSE.

Special FeaturesWhats it all about?
  • What is Special Features?
  • It is the 4th task button on the green Menu Bar.
  • The place to Not Give several doses at the same
    time and attach the same Not Give reason to all
    of them. Handy if the patient was off the unit
    for awhile and many doses were Not Given in
    their absence.
  • This is also the only place to Remove doses.
    The removed doses are given the unknown
    status on the MAR View.
