  • All equipment provided carries risks as well as
    benefits.
  • Risk Assessments must be carried out and
    documented as per local procedure.
  • Some equipment must only be provided with
    evidence of risk assessment.

  • As the Prescribing Clinician it is your
    responsibility to ensure that
  • you have read all the criteria for provision of
    the equipment you are
