Mishap Report
  • Mishap description On Oct 19, five minutes into
    a flight, the engine suddenly died. The
    instructor took control of the flight and glided
    it to a uneventful landing.

  • Investigation and Cause Human error, preflight.
    The pilot and mechanic were convinced that the
    problem was electrical in nature and focused
    their investigation on the magneto. After
    several hours the fuel tank was checked and found
    to be empty. The pilot admitted responsibility
    when he said, The joke is on me boys. The bird
    wont fly with out gasoline.

  • We all know that the CAP SAREX is a call to
    practice skills
  • We practice because we want to be able to respond
    appropriately in an emergency
  • Why not incorporate a mishap scenario into your
    next SAREX
  • Reviews procedures

    information to collected, on-line form 78
  • Identifies limitations

Sentinel Update on Applications for CAP/SE
Support Staff
  • Call for applications to serve on the volunteer
    support staff for the National Safety Officer is
    forthcoming
  • Do not start sending in resumes or letters of
    interest.
  • Expect announcements for flight safety,
    transportation safety, and personal safety, i.e.
    bodily injury, specialists to assist the National
    Safety Officer.
  • They will let us know when they are ready for
    applications.

Sentinel Safety Management Systems (SMS)
  • SMS is a heavy reliance on data that is, in turn,
    used to help guide senior leader decisions across
    the enterprise
  • Expect more emphasis on data collection and
    relevant analysis to help reduce risk and support
    mission success
  • SMS is a quality management approach to
    controlling risk (FAA has an advisory circular
    that covers SMS)

The four pillars of safety management!
  • The SMS is a management standard that is
    organized around four basic tenets of safety
    program management
  • Policy CAP is a military-style organization
    and the Air Forces auxiliary. We have plenty of
    policies and sometimes we just need to look
    around, remind ourselves of existing guidance,
    and make sure we comply!
  • Safety Risk Management ORM fits the bill and
    thats all well say about that for right now.
  • Safety Assurance This is the one piece that
    CAP needs to work on most, and we will construct
    that pillar in the coming months.
  • Safety Promotion This is not just a slogan or
    a campaign, but a commitment to a Core Value
    (like Excellence and Integrity). Why reinvent the
    wheel when its sitting out in the front yard
    already?

    mishap prevention?
  • Learn as much as you can about Critical Incident
    Stress Management.
  • Learn how to recognize signs of distress and
    where to turn for help.

Sentinel How to Practice Reporting Mishaps
  • Simulating the submission of a mishap report
  • Under no circumstances should your simulation
    include entering data into the on-line safety
    system
  • Use a printed form 78 from the Safety Forms web
    page in e-Services
  • If making the actual contacts listed in the MN
    Wing Supplement, ensure everyone understands it
    is an exercise

Annual Safety Day
  • CAPR 62-1 4 JUNE 2008 Para 3.h An annual Safety
    Day will be held in all units sometime during the
    month of October to focus on improving safety
    knowledge and attitudes during the new fiscal
    year.

Deer Season
  • All zones of MN (including the Metro) have
    seasons
  • Deer hunting November 7 to 15th (some zones are
    active up to 29 Nov)
  • Hunters have their upper body and head gear in
    Blaze Orange
  • CAP ground teams could be miss-identified if not
    properly clothed
  • Recommend same Blaze Orange gear as hunters
    (Leave the antler hat and camouflaged gear at
    home!!!)
