  • (3) How to promote IVM

4
Integrated Vector Management Brief Historic
Review
  • Integrated Vector Control (IVC)
  • the utilization of all appropriate
technological and management techniques to bring
about an effective degree of vector suppression
    in a cost-effective manner
  • WHO 1983

Integrated Vector Management Brief Historic
Review
  • Integrated Vector Control (IVC)
  • the utilization of all appropriate
technological and management techniques to bring
about an effective degree of vector suppression
    in a cost-effective manner
  • Elements
  • economics
  • vector control technology
  • output indicator is vague

Integrated Vector Management Brief Historic
Review
  • Selective Vector Control
  • the targeted use of different vector
control methods alone or in combination to
    prevent or reduce human-vector contact
    cost-effectively, while addressing sustainability
    issues
  • WHO, 1994

Review
  • Selective Vector Control
  • the targeted use of different vector
control methods alone or in combination to
    prevent or reduce human-vector contact
    cost-effectively, while addressing sustainability
    issues
  • Elements
  • economics
  • sustainability
  • flexibility
  • output indicator transmission risk level

8
Integrated Vector Management Brief Historic
Review
  • Integrated Vector Management
  • A process of evidence-based decision-making
procedures aimed to plan, deliver, monitor and
    evaluate targeted, cost-effective and
    sustainable combinations of regulatory and
operational vector control measures, with a
    measurable impact on transmission risks,
adhering to the principles of subsidiarity,
    intersectoriality and partnership
  • working definition, 2001

9
Integrated Vector Management Elements of IVM
  • Its about process, not contents
  • It spells out the need for an evidence base
  • it retains economics and sustainability as key
    criteria
  • it mentions both regulatory and operational
    measures
  • it further defines the output indicator
  • it embraces three principles, relating to health
sector reform, and action outside of the health
    sector

10
Integrated Vector Management Elements of IVM
  • Evidence brings in the issue of ecosystem
    analysis as a starting point
  • This in turn leads to a logical build-up of
    integrated measures, from environmental
    management methods, through biological to
    chemical methods of control
  • An IVM approach invites the re-consideration of
the conventional boxes of chemical, biological
    and environmental control
  • Leaving chemicals as a measure of last resort
recognises their value as a resource that needs
to be sustained as long as possible.

11
Integrated Vector Management Elements of IVM
  • A regulatory role and the concept of
subsidiarity raise the issue of where IVM fits
  • In the health sector or in the environment sector
    ?
  • In the core of the health sector (with disease
    control programmes), or
  • At the boundary, where it can better interact
    with other sectors?
  • How are essential functions going to be divided
    over the
  • various levels (national, provincial, district) ?

12
Integrated Vector Management How to proceed
with IVM ?
  • WHOs normative role in vector control
should be extended from technical tools and good
technical practice to include good practice in
    managing vector control programmes
  • WHO must start a process of mainstreaming
the concept of IVM with a view to making it its
core strategy for the next 10-20 years.

13
Integrated Vector Management How to proceed
with IVM ?
  • WHO must start a process of mainstreaming
the concept of IVM with a view to making it its
core strategy for the next 10-20 years.
  • This will require a process of consensus
    building in the Organization itself.
  • It also requires stakeholder consultation
outside the Organization to get other partners on
    board

14
Integrated Vector Management How to proceed
with IVM ?
  • The delivery of this concept to the Member
States should take advantage of the currently
ongoing processes of health sector reform,
    including de-centralisation

15
Integrated Vector Management How to proceed
with IVM ?
  • Assistance to Member States may consist of
  • (1) regional workshops promoting key IVM
    principles such as cost-effectiveness analysis
    (with follow-up)
  • (2) assisting a small number of truly interested
countries in the process of completely
overhauling their vector control programme and
serve as a model in their Region
  • (3)detailed documentation of all experiences for
analysis and dissemination to all Member States
  • (4) capacity building

16
Integrated Vector Management Capacity
building
  • Carry out a needs assessment
  • Redesign the vector control programme and its
    essential functions
  • Prepare a blue print for decision making criteria
    and procedures
  • Review options for regulation
  • Organise workshops on cost-effectiveness analysis
    of vector control options
  • Establish or strengthen arrangements for
    intersectoral collaboration
