Step 1 Create a Base Sheet
  • First, you need to input data into the workbook.
This can be done in two ways
  • Create a template for the Base Sheet
  • which sets up the required data fields.
  • Paste data into this worksheet.
  • Input your facility DAD data into your
  • own worksheet and convert that
  • Worksheet to the Base Sheet format
  • (recommended option)

5
Converting your DAD worksheet
  • Create a DAD worksheet including the required
data. Ive called the worksheet MyDAD

6
  • Clicking
  • leads to the form enabling you to match the
required fields to the column identifiers in your
DAD worksheet. Choose the column identifiers from
    the drop-down lists.

7
leads to a new Base Sheet
8
If you have ALC data to analyse
  • Click this button to add ALC information

9
which leads to the addition of the following
columns to the Base Sheet
  • Note that ALC calculations (for the monitoring
indicator) require that all DAD data for the
relevant period are included in the Base Sheet

10
End of Step 1
  • The following additions are now in the
    Summary Statistics worksheet

11
Step 2
  • Worksheet A is created to hold the data for
readmission index cases and creates the necessary
    fields for indicator calculation.
  • You are taken through a number of options and
information points during the creation of this
    worksheet

12
Worksheet A
  • A number of popup boxes ask for
  • or provide information

13
End of Step 2
  • Now there is a Worksheet A, another sheet with
the deleted records and an updated summary in the
    summary worksheet.

14
Step 3
  • adds statistics for readmissions and LOS

15
Step 4
  • This creates a Charting worksheet to enable
    graphing control charts
  • Choose the dates for the charting period.
  • The Odds Ratio for the CUSUM chart can be left as
    2

16
Step 5
  • Control charts for all readmission cases and
individual cohorts (with gt 50 cases) are plotted

17
Notes
  • These calculations are based on the DAD data to
2004/5. Updated analyses based on data to 2005/6
will be included in the next version (Version 2)
    of the calculator.
  • System integration indicator calculations will be
    included in the next version
  • All dates ought to be entered as strings in the
    yyyymmdd format (e.g. 20041231)

18
Notes about data fields
  • The RIW exclusion factor should be
  • 0 for typical cases
  • 1 for outliers
  • other value for deaths, signouts, transfers
  • The Institution To Type and Discharge Disposition
fields are not used yet they will be used for
    the System Integration indicator
  • The Total LOS is used for the Explanatory
indicator (related to readmissions), and for the
    ALC indicator
  • The ALC Days is used for the ALC indicator

19
Notes about Data Fields
  • Institution to Type
  • We are interested in whether this is 8 home
    care

Discharge Disposition We are interested in
whether this is 04
20
Notes
  • For the ALC indicator, all inpatient records for
the period of interest ought to be included in
    the Base Sheet.
  • Sometimes, the control charts for the cohorts
appear be overlapped. This often happens when you
first re-open the file. Rerunning Step 5 once or
twice to regenerate the charts solves this
    problem.

21
Notes
  • The calculator is programmed in Visual Basic. It
was developed in Microsoft Office Excel 2003. We
have attempted to make it compatible with Excel
2000 some features (e.g. tab colours) may not
    appear
  • This version has been run without errors in the
Excel 2003 environment. If errors appear when you
run the program, you can try the following

22
If errors arise.
  • Check that your data are in the right columns and
    in the correct format
  • Try unprotecting worksheets and rerunning the
    problem step.
  • Try running on an Excel 2003 version if you have
    access to one.
