Horizon Patient Folder is the hospitals legal
electronic medical record repository. The
patients medical record is scanned or
automatically transferred through a COLD
(Computer Output to Laser Disk) process to
pre-defined location. All scanning is performed
by the Health Information Management department.
Horizon Patient Folder is also where Physician
deficiencies are assigned so they can display for
the Physician in Horizon Physician Portal.

Horizon Practice PLUS Clinic Registration
Horizon Practice PLUS is a Scheduling and
Registration system that is used in the Zimmer
Outpatient Clinics (OB/GYN, Med/Surg and Peds).
This is used by the Physician and Nurses in the
Clinic to view the Clinic schedule of what
patients they will see on a given day. It will
interface with Horizon Ambulatory Care, the
electronic medical record to be implemented in

DAS Automatic Recording for Patient Monitors
NOW
There are two components of the DAS, the NOW and
the Confirmation Queue (CQ). When a clinician
selects the NOW button in HED, DAS takes a
snapshot of the vital signs and populates the
right-hand side of the screen.
In addition, every 60 seconds, DAS will take a
snapshot of the vital signs and store them for a
24-hour period. This is the CQ functionality of
DAS. The clinician has the capability of
reviewing all of this data and either selecting
all of the values or individual fields to be
saved to the patient record. The CQ is located
on the left-hand side of the charting screen.
