What happened NM - DRILL COLLAR SLIPS THRU
ELEVATORS The drill crew was picking up and
running BHA (bottom hole assembly) components.
The elevators were lowered and latched on to the
lifting sub of a 6 ½ heavy weight drill collar
which was sitting on the hydraulic carrier. As
the collar was raised with the top drive to
approximately 20, the lifting sub slipped out of
the elevators. The collar fell against the
draw-works guard cover and the pin-end slid off
the hydraulic carrier to the ground level,
striking the HPU power source cable. The lifting
sub end of the collar stopped at the top of the
V-door.

- Elevators were not changed when 8 BHA tools
changed to 6 ½ drill collars . - The scope of the JSA Tripping in Hole w/ BHA
should - Address specific OD changes in tools
- Reference JSA Changing out Elevators
- The JSA Pause process should be used when changes
in job scope or unexpected delays require
reassessment of risk. - Immediate Corrective Actions Taken
- Color coordinated elevators and lifting subs to
prevent potential mismatches in the future. - Re-emphasize policy of measuring OD on all lift
subs prior to task performance
