Warfarin patient stories

• Aim
• To get the patients perspective of the
  anticoagulation services available in NPT
• Advice sought from the LHB Patient Public
  Involvement Group and an advert was placed in the
  local newspaper

Collecting our story

• LHB Head of quality patient safety and LHB
  pharmacist collected the patient stories
• GP practice identified 2 patient volunteers (1
  patient from each anticoagulation monitoring
  service)
• Both patients interviewed at the same time
• Session recorded for analysis

The outcome

• Conclusions
• One service model does not fit all
• Patients may need more education about the
  anticoagulation services available to them
• Briefing paper taken to LHB clinical governance
  meeting
