    National Council of Aging
  • Objectives
  • Pilot the Healthier Living with On-going Health
    Problems software developed by Stanford
  • Measure health behaviour outcomes
  • Evaluate preference of on-line to in class
    workshops
  • Assess user experience/friendliness
  • Determine if on going program would be valuable

What is the On-Line Program?
  • 6 week, interactive online workshop
  • Each workshop will have roughly 25 participants
  • 2 peer Stanford On-Line Trained moderators per
    workshop
  • Participants log on at their convenience 2-3
    times per week for a total of about 2 hours per
    week
  • Participants do not need to log on at the same
    time
  • Participants receive Living a Healthy Life with
    Chronic Conditions Book on-line to download
  • Questionnaire administered at beginning of
    workshop, 6 months and 12 months
  • On-line informed consent and authorization to
    store personal information are completed by each
    participants

Software Details
  • The on-line workshop offers 4 distinct areas
  • The Learning Centre educational content/lessons
    are delivered each week
  • The Discussion Centre messages are exchanged
    between participants and moderators
  • My Tools each participant creates a work plan
    with specific goals and objectives for the coming
    week
  • The Post Office for private one to one
    communication between participants and moderators

Timelines
  • Recruitment of moderators and participants
    Currently under way
  • Training of moderators (6 week course 10 hours
    on line) January
  • First workshop Early March

Recruitment
  • Recruiting 20 moderators from across Canada
  • Must be an experienced in class trained lay
    leader
  • Must be computer competent have access to high
    speed internet access
  • Training Will first take part in the 6 week
    on-line work shop the Stanford On-Line
    Moderator Training (10 hours)
  • Workshops Must be able to commit 10 hours per
    week to the on-line workshop for 6 weeks
  • Moderators will receive an honorarium of 400 CDN
    per workshop
  • Recruitment of patients Looking for web sites to
    link to that would attract patients i.e. on-line
    diabetes education website
  • Recruiting 300 participants
  • Over the age of 18
  • Have a chronic Illness
  • Have access to the internet
  • No cost for participation
