• Population projections used for quantification
  rather than health seeking behavior
• No standard operational procedures in place at
  storage facilities
• SRs responsible for utilization data reported
  late and incompletely
• Tertiary facilities did not report back on use
• Large number of final service points made data
  retrieval difficult
• Slow start of grant forced a compression of 2
  year targets into 9 month time frame

• Initial push system was not adapted to usage
  information
• Inadequate data concerning use
• Wide spread stock outs
• Failure to achieve program objectives
• Reprogramming of the grant

Steps Taken

• SOP manuals developed
• Supply chain management sub-contracted to CHAN
  Medi-pharm
• Standardized documents developed and put into use
• Treatment documentation aligned with distribution
  and re-supply documents
• Program managers collect usage data and authorize
  re-supply
• Procurement agent hired to assist in PSM and
  capacity building.
