NDIS Specialist in Victoria
Journey Health Solutions provides a high quality,
personalized support services to people with a
disability aged 18-64 in Victoria. We are a group
of dedicated health professionals who have been
working for many years treating and supporting
people with a wide range of intellectual,
physical, sensory and mental health disabilities.
We understand that most people require different
types of help from different professionals at
different times in order to achieve their goals
and maximize progress. Thats why we offer a full
range of support services, allowing you to meet
all your needs in the one place.

Social Community Participation
Let us help you to take part in community
activities and social events. This support is
great to help you feel connected to others and
the broader community and to prevent social
isolation. Our staff can support you to attend
any activity that you chose whether it be
one-to-one or in a group setting. Some examples
of the types of activities our clients have been
supported to attend include Groups or
classes Sporting events Seeing a movie Going to a
café Live music events Cultural and religious
events Visiting a friend or relative This support
can also include help with managing anxiety and
building your social skills and confidence around
others.

Support Co-Ordination
Support Co-Ordinators at Journey Health
Solutions, will assist you to work towards your
identified goals in your NDIS plan. This will
involve navigating the NDIS service system,
making informed decisions and obtaining the
resources and support that you need and
want.Using a person-centred and recovery focused
approach, your Support Co-Ordinator will plan,
develop and coordinate services for you
by Liaising with internal and external services
to maximise participant outcomes. Planning,
developing and building capacity in individuals
and assist you to manage your own
supports. Monitoring and reviewing service plans
and goals in consultation with you on an ongoing
basis. Providing support and assistance as
appropriate to relevant parties. Assist the you
to access and understand your NDIS plan.

Behaviour Support
Many people with disabilities experience
behavioural difficulties that require expert
assessment and intervention in order to live the
life they want and make sure themselves and
others are safe. At Journey Health Solutions we
have professionals who can provide a range of
services to support the management of challenging
behaviours and create positive change. These
services may include Specialist Functional
assessments to determine target behaviours of
concern and assist to develop positive, evidence
based interventions for reducing
these. Development of comprehensive Behaviour
Support Plans that aim to limit the likelihood of
behaviours of concern increasing or prevent them
from developing. Training and assistance to
families, carers or other support staff in
implementing behaviour support strategies.

Personal Care
Personal care supports are designed to assist you
with personal tasks that will help you to live as
independently as possible.  We work with you,
your family, carer or other support people to
accurately assess your needs and develop a plan
that works for you. Some examples of the types of
personal care tasks we might assist with
include Bathing Personal hygiene and
grooming Dressing Feeding Toileting Oral and
denture care We understand that everyone has
different personal care needs and will develop a
personalised plan to meet your unique
requirements.

Therapeutic Supports
Occupational Therapists can provide assessments
and interventions to help you find ways to
complete tasks more easily. This may include
identifying issues with strength or concentration
and finding the right equipment or modifications
required in your home or workplace to achieve
your goals. They will assess how you are going
currently and develop a plan for you, your
family, carers or other support people to be able
to put in place to make life easier and help
build your independence. The types of assessments
our OTs can provide include Support needs
assessment Assessment of independent daily
living Functional behavioural assessment Sensory
processing assessment Our OTs can also assist you
to purchase and successfully use assistive
technology and other therapy products you may
require. They can also work with other
professionals to develop Behaviour Support plans
for the management of challenging behaviours.

Nursing Care
Community nursing care is available for services
that are related to your disability and
contribute to your long-term wellbeing. Our
nurses can provide support and clinical
interventions to meet your healthcare needs. They
can also provide training to enable you, a family
member, friend or support worker to undertake
supports so that these can be achieved without
nursing assistance. The types of service our
nurses provide includes Health care assessments
including continence assessments Clinical
interventions such as woundcare, catheter and PEG
changes Health related training such as
medication prompting and wound care Support to
access to a GP or other health professional
