Circumstances of the Case
  • Ms. Rafael, a thirty tree-year old mother came to
    Provincial Social Welfare Development Office on
    July 11 accompanied by Mrs. Em, her common law
    husbands sister in law. The client and her
    family had been staying with Mrs. Ems family in
    an abandoned building for years. Looking upset
    and shy, Ms. Rafael verbalized that she is
    pregnant and is insensible of how many months is
    her baby. She also added that sometimes she
    faints instantly and is worried since she could
    not feel any responses from the baby on her womb.

II. BACKGROUND INFORMATION
THE CLIENT
Ms. Rafael is 33 years old and a bonafide
resident of Mangandingay, Cabarroguis, Quirino
for years. The client is living with her common
law partner Mr. Capalar and was gifted with three
children who are not registered at the Local
Civil Registrar and Philippine Statistic
Authority. Presently, Ms. Rafael verbalizes that
she is pregnant and is insensible if how many
months old is her baby. The client stated that
she only finished grade -2 and is not capable of
math basics and even reading the calendar.
Likewise, the client has not visited the doctor
yet for her prenatal check-ups and does not take
vitamins for her pregnancy.

II. BACKGROUND INFORMATION
1. THE FAMILY
The family is living in an abandoned elementary
school at Mangandingay with her common law
husband brothers family where they use
recyclable materials such as cartons and sako
bags as a covering division of the building that
serves as their room. The buildings
surroundings is unclean covered with tall grasses
and is not convenient for housing since it had
been destroyed leaving it with uncovered doors
and windows free from trespassers and wild
animals.

2. THE FAMILY
With the given statement of the client that
sometimes they only eat meals twice a day, worker
discovered that both of common law partners dont
have a permanent job since they only attained
elementary education.
Her common law husband is a daily laborer who
earns P200.00 a day where there is no assurance
of income because it depends if there is an
offered job by their neighbors while, the client
is staying in the house to take care of her
children.

THE COMMUNITY
The family is living in a peaceful neighborhood
despite the inconvenient location of the house
is surrounded with tall grasses. Also, the
family has good relationship to their relatives
and selected people whom they mingle with.

According to Karl Jungs theory of personality
those people who are quite and prefer to be in a
smaller group has the Introvert type of
personality in which they tend to be unexpressive
of feelings and are not socially inclined.

THE CLIENT
Ms. Rafael doesnt seek opportunities to earn an
income in order to help her husband for the
provision of their daily basic needs. She only
finished grade 2 which made her incapable of math
basics like addition and subtraction and even the
calendar reading leaving her insensible on how
months old is her baby.

III. ASSESSMENT
THE FAMILY
The clients family is considered as
dysfunctional family since they could not well
provide for the basic needs of their children and
they are not performing their reciprocated roles.
Though, the relationship of the common law
partners with their children is good.
Furthermore, during the worker house visit, she
observed that the children help the client in
doing household chores with respect and good
attitude towards the client. The clients
family has no means of income and savings and
even personal properties such as land and house.
Their relatives help them but are not constant
since they have their families to support too..

III. ASSESSMENT
THE COMMUNITY
Resources in Ms. Rafaels immediate problem is
available in the community since theyve got
Rural Health Unit that conduct prenatal check-ups
for pregnant women and also distributes vitamins
for free. It is only the client who hesitates to
go because of shyness hiya and her personality
as stated above. At some reasons, the client
lacks of for transportation going to the
RHU. With regards to their participation and
activities being undertaken in the barangay, the
common law partners are not so active since their
time is devoted into looking for job
opportunities just to earn a living. And also
with the stated personality of the client being
an introvert who prefers to stay at home affects
their in active participation in community
activities.

The worker views not just the clients immediate
need but also the clients need of the family
general.
The worker addressed and directly provides the
need of the client (medical Care)
The worker involves the process of negotiating
the service jungle for clients. The worker
connects the client in the system until he has
availed of them.

V. ACTION TAKEN
  • Intake interview and assessment of needs.
  • Help her avail medical assistance under the
    Crisis Intervention Program and free medical
    check-up for her pregnancy at the Rural Health
    Unit of Cabarroguis.
  • Link client for Pelvic ultrasound at QPMC charged
    under the medical assistance of the PGU
  • Link client for free vitamins from Rural Health
    Unit of Cabarroguis
  • Assist her on her follow-up check-ups
  • Advise giving and agreement on keeping her
    pregnancy safe attending pre-natal check-ups
    until giving birth
  • Conduct of advice giving sessions re responsible
    parenthood and maternal care and child health
  • Link the common law partners with the municipal
    registrar for mass wedding

V. EVALUATION
  • Based on the foregoing case management, client is
    attending pre-natal check-up with the social work
    interventions extended to the client but is not
    consistent. She is now into on-call labor such
    as laundry and weeds cleaning to augment their
    income for their daily basic necessities and able
    to realize the importance of prenatal for the
    welfare of her health and so with the baby.

V. RECOMMENDATION
  • With this, worker highly recommended that Ms.
    Rafael must be supervised to consistently attend
    regular pre-natal check-up and regular vitamin
    intake to maintain her normal health condition as
    well as her baby and her children.
  • As to the children, the worker recommends that
    they must be registered at the Municipal Civil
    registrar for future purposes.
  • Furthermore, the family must be referred to a
    core-shelter project to at least have their own
    house and lot for a convenient place to live-in.
