must be signed by you in the presence of a notary
public and two witnesses. You can revoke this
document anytime by destroying it, revoking it in
writing, and or creating and executing a new set
of Instructions

wishes agrees, it is expected that s/he will
follow through on your instructions when the time
comes to celebrate your life after you pass.
