4When to use the EAP
- Often, stress caused by life changes, conflict or
trouble at home or at work may make it difficult
for you to do your job or to be happy in other
areas of your life. - Employees are encouraged to seek help through the
EAP when they are having problems which they are
unable to resolve on their own. - The best time to seek help is before the problem
elevates to a crisis level.
5How many visits are covered under the EAP and
what is the cost?
- Up to 6 EAP Visits per contract year at NO COST
Per Employee and Eligible Family Member. - Group Health Plan Participants Requiring Longer
Term Care Under the Plans Mental Health
Component - Transition to the Plan and Incur Only a
Co-payment for That Continued Treatment - Not Covered Under a Group Health Plan
- Transition to Community Resource, Self-help
Groups, etc.
6What happens when you call your EAP?
- When you contact your EAP at 1-800-272-3626 a
licensed professional is always available for
crisis intervention. - For routine requests/appointments, a CSR will
gather information such that includes specific
provider criteria such as location, day and time,
availability and specialty. - An initial appointment will be scheduled with a
licensed professional. - The network provider will conduct a formal,
face-to-face assessment at your first session.

8Mental Health Substance Abuse MH/SA
- All Health Plan Participants
- Eligible for the MH/SA Benefits
- Benefits Are Per Contract Year
- January 1 Through December 31
- All Medically or Clinically Necessary MH/SA
Services and Conditions Shall Be Covered for the
Period of - Evaluation
- Initial (Short Term) Stabilization
- Case Management
- Referral

- Up to 31 days
- 200 Copayment per admission.
- Out-of-network emergency coverage only
- OUTPATIENT
- Up to 60 visits
- 10 copayment per visit
- Out-of-network paid at 80 of network fee schedule
