- NYS Options for People Through Services - OPTS
FOR ONE - An individual can work with a voluntary provider
to design an innovative and individualized
program to meet his/her specific service and
support needs - On behalf of that individual, the agency develops
a proposal specific to the applicants interest - Once the plan is approved, OMRDD contracts with
the agency to carry out the services and supports
for the individual as specified in the proposal

- Getting Help What is a Start-up Broker?
- Families and MSCs often dont have the time or
ability to do all the tasks associated with
getting a SD/CSS application and plan completed. - Startup Brokers are independent Contractor hired
by the person for a negotiated price based on the
persons needs (100 state funded) - Assists the person to develop the
Self-Determination/Consolidated Supports and
Services (CSS) application and initial SD/CSS
service plan - Tasks are separate from those of your Medicaid
Service Coordinator
