PERFORMANCE PLAN FOR DIRECTORATE   SECTION A
(KPA's) Step 1 Weight Please provide a
percentage for each Departmental KPA in the space
provided at "Weight".   Step 2 Key Performance
Indicators 3rd column (Key Performance
Indicators).   Note that columns 1 and 2 was
extracted from the approved IDP

Step 3 Baseline Refer to column 4 "Baseline"
and provide us with the necessary
information. "Baseline" is defined as
follows The information obtained when a
municipality for the first time collects
information for any measure. Targets are set in
relation to the baseline.   Step 4 Quarterly
Targets Please provide quarterly targets for each
of the KPI's in the 4 blocks provided. The
targets should preferably only be a number and
must relate to the KPI.   Step 5 Evidence In
the last column provide the evidence that will be
provided at the evaluation sessions to
substantiate the meeting of targets.

SECTION B (CCR's) Contains all the CCR's as
stipulated in the "Regulations " . According to
Regulation 26(8), the CCR's should be selected
from the list, as agreed to between you and the
