Benefits Specialist Receives Online Sickness
Accident Notification from HR Officer on the
8th calendar day of absence or Notice of
Controverted Claim from Workers Compensation
Ben Specialist sends out Short Term Disability
Letter, DB-450 and the Employee Health
Examination Report (EHER) to employees home
On the DB450, employee completes Section
A healthcare provider completes Section B
Employee sends DB450 to Benefits Office
Employee does not supply properly completed DB450
If the DB450 isnt returned after two weeks,
Benefits sends first reminder letter to employee
with cc to HR
Benefits Office sends DB450 to Zurich Zurich
verifies illness and length of absence
If after ten calendar days, the employee does not
supply documentation, second (final) reminder
sent with a copy sent to the HR
Benefits Office sends HR email with expected
return date
Benefits advises HR if illness is denied by
Zurich HR contacts Employee Relations
for guidance on next steps
When sick time runs out HR Rep must send PAF to
put employee on leave type FS if FMLA applies or
IL if it does not apply
