2Your shipping kit should include the following
supplies
- One insulated (Styrofoam) container
- One Sarstedt cool transport container with small
outer Styrofoam case. Place the plastic transport
container flat in the freezer to freeze. - Cold Pack
- 2 bundles of tubes 2 red top tubes and 1 green
screw top transfer tube - 2 plastic pipettes
- One 6x 9 brown envelope labeled Please read
instructions carefully - Patient Instruction Sheet
- 2 HIV consents
- One prepaid FedEx return airbill

4Everything that comes in the kit needs to be
returned. Do not discard or dispose of materials.
There are enough supplies in one kit to collect
and send Day 3 and Day 10 blood work or two sets
of communicable testing lab work.
Take all of the supplies and paperwork with you
to the lab that will be drawing your blood.
If you use a shipping kit supplied by Fertility
Labs of Colorado there is no need to purchase or
use dry ice. The shipping kit contains all the
supplies you will need.
5Collection of Blood
- Collect the blood in two plain red top tubes. DO
NOT USE SERUM SEPARATOR TUBES. - All specimens (Red Top Tubes and Transfer Tubes)
must have the following information on them - Patients full name (First and Last)
- Last 5 digits of social security number
- Date of birth
- Date and time of collection
- Allow the blood to clot completely. Approximately
15 minutes. - Centrifuge the samples and use the plastic
pipette to separate the serum from the red blood
cells. - Place the serum into the transfer tube and
dispose of the red top tube. Please do not send
the whole blood back with your kit. We only need
the serum portion of the blood.

7Do not ship us whole blood. If you do, your
sample will be rejected and you will have to
recollect.
We test the serum portion of the blood. It is a
light yellow liquid. The red blood cells can be
discarded.

10The Correct Way to Repack a Shipping Kit
- Freeze the serum specimen(s) for approximately 24
hours prior to shipping. Samples are to be frozen
before being placed in the Sarstedt container. - Place the frozen transfer tube(s) containing the
serum in the previously frozen Sarstedt transport
container. - Place the Sarstedt container in the small
Styrofoam case. Close the top and secure it with
the rubber band. - Place the closed Styrofoam case into the large
Biohazard bag. - Enclose the patient instruction sheet and any
consent forms in the outside pocket of the
Biohazard bag. - Place the Styrofoam case which should be sealed
inside the Biohazard bag inside the larger
Styrofoam container lying on its side. Place the
gel cool pack into the remaining space. - Replace Styrofoam container lid and seal
cardboard box back up - Place the prepaid FedEx air bill to the outside

- DO NOT SEND SAMPLES ON FRIDAY FREEZE AND HOLD
UNTIL MONDAY TO SHIP. - DO NOT WRITE DIRECTLY ON THE OUTSIDE OF THE
SHIPPING BOX. - DO NOT WRITE DIRECTLY ON ANY OF THE STYROFOAM or
OTHER PACKAGING MATERIALS. - BE SURE TO HAVE A WITNESS SIGN THE HIV CONSENT
FORM IF YOU ARE HAVING COMMUNICABLE DISEASE
TESTING DONE. - DO NOT TAPE CLOSED ANY OF THE STYROFOAM
CONTAINERS. - EVERYTHING THAT IS SHIPPED TO YOU IN THE KIT MUST
BE RETURNED TO US. REPLACEMENT COST OF MISSING
