- Risk Factors (relapse, dropout)
- Software for clinical integration
Polaris systems can help differentiate provider
organizations. The information Polaris systems
collect can fundamentally change service
decision-making and improve outcomes and
cost-effectiveness.

5Documenting Outcomes On-Line Aggregate Analysis
Site A 03/05/03 to 09/10/04
My life has meaning and purpose.
6The graph below shows the distribution of
patients whose Update PSYCHIATRIC severity
(percentile) score, compared to Intake, was
better, the same, and worse. The graph includes
only patients who (a) received treatment at
site A, (b) took an Intake between 04/06/01 and
09/24/04, (c) had an Intake PSYCHIATRIC severity
(percentile) score greater than or equal to 25,
and (d) took an Update between 0 and 60 days
after Intake. (For patients who took more than
one survey in this time period, the graph
includes only the survey that is closest to the
mid-point of the time period.)
