  • Confirm that employer has HSA eligible HDHP
  • Discuss the appropriate enrollment process for
    the client
  • Discuss and confirm the appropriate contribution
    process for the client

HSA Enrollment Options for Employers
  • Web enrollment (Employee)
  • Employer receives customized Web Enrollment
    Instruction sheet
  • HSA Employee completes online enrollment to
    establish account
  • Employee receives customized kit
  • The Welcome Kit will be mailed to the employee
    within 7-10 days of receiving the completed
    application
  • Batch enrollment (Employer)
  • An electronic file with the necessary employee
    data is sent to the U.S. Bank Implementation Team
  • A pre-filled enrollment kit will be mailed to the
    employees home within 7-10 days
  • Employees receives customized kit
  • The Welcome Kit will be mailed to the employee
    within 7-10 days of receiving the completed
    application
  • Paper-based enrollment (Employee)
  • Employer receives Enrollment Kit electronically
    (soft copy) and distributes to HSA Employees
  • Employee reviews product information and
    completes HSA application.
  • Employer batches completed applications and sends
    them to U.S. Bank

Step 3. Employee Enrollment
  • Employees complete the enrollment process
  • U.S. Bank will mail HSA Welcome Kits 7-10
    business days after receipt of completed
    application
  • Welcome Kit includes HSA check stock, deposit
    slips and resources regarding employee
    interaction with their account
  • VISA debit card will arrive in a separate
    mailing shortly after Welcome Kit

  • U.S. Bank Implementation Team works with the
    employer to integrate selected contribution
    process
