What is FAMIS?
  • FAMIS is Virginia's health insurance program for
    children.
  • FAMIS provides access to quality health services
    for children.

What does FAMIS Do?
  • FAMIS covers all the medical services growing
    children need to avoid getting sick, plus the
    services that will help them make a speedy
    recovery if they do become ill or get hurt.

  • Tests and x-rays

How Much Does FAMIS Cost?
  • There are no enrollment costs or monthly premiums
    for FAMIS.When your children get care there may
    be a small fee called a co-pay for some services.
    Most co-payments are just 2 or 5 - FAMIS covers
    the rest. You do not pay for well-baby or well-
    child checkups.

Who is Eligible for FAMIS?
  • Your children may be eligible if they-Live in
    Virginia -Are under age 19 -Don't have health
    insurance now and haven't had it in the past 4
    months (see www. FAMIS.org for exceptions) -Are
    not eligible for the Virginia state employee
    health insurance plan

  • Your children may be eligible if they
  • -Are not eligible for FAMIS Plus (also known as
    Medicaid) -Live in families meeting FAMIS income
    guidelines-Are United States citizens or
    qualified aliens (other children may be eligible,
    please call to find out more)

Income Guidelines
  • Qualifying for FAMIS also depends on household
    income and family size. To find out your family
    size, count how many children and stepchildren
    under age 19 live in your home. Add yourself if
    you are the mother, father or stepparent of the
    children. Then add your husband or wife. For
    example, if you have 4 children and/or
    stepchildren under age 19 living in your home,
    plus yourself and your husband, you have a family
    size of six people. The monthly income limit
    for a family of six is 4,312.00.

How to Apply
  • It's easy to apply for FAMIS. You will need to
    complete a Health Insurance Application for
    Children and Pregnant Women. You may do this
    online, over the phone, or by mail or fax. You
    will need to share information on who is in your
    family and your family's income.

Online Application
  • You can apply online! The application will take
    you about 20 minutes and can be completed from
    the privacy of home. When you complete the online
    application, you will need to print out a
    "Confirmation Page," sign it (your signature is
    required), and mail or fax it in with any needed
    documents.

  • Children on FAMIS will have health insurance
    coverage for 12 months
  • At the end of the 12 months the FAMIS coverage
    needs to be renewed

  • The FAMIS MOMS program provides health care
    coverage for pregnant women. Good health care
    during pregnancy is important for you and the
    baby.
  • FAMIS MOMS encourages pregnant women to get early
    and regular prenatal care to increase the
    likelihood for a healthy birth outcome.

FAMIS MOMS Eligibility Guidelines
  • The mother applying must
  • Currently be uninsured
  • Meet family income guidelines
  • Have a medically confirmed pregnancy or be within
    the sixty-day period after your pregnancy ends
  • Not have access to the State Employee Health Plan
  • Be a resident of Virginia
  • Be a U.S. citizen or a qualified legal immigrant

  • 100 per month per child up to the total amount
    of the family premium
  • Children must be eligible for FAMIS
  • Employer-based insurance or private policy
  • No wrap-around services
  • No co-pay, deductible, or co-insurance
    reimbursement
