About AFBA
  • Armed Forces Benefit Association (AFBA) was
    established in February 1947, under the direction
    of General Dwight D. Eisenhower.
  • Since the founding of AFBA, its purpose has been
    to provide low cost insurance and financial
    services for men and women in the U. S. military
    during peace and war.
  • (most commercial insurance companies would not
    cover wartime risks in 1947)
  • Since then, AFBA has expanded the eligibility.

  • Since 1947, AFBA has paid in excess of 970
    million in death claims to the beneficiaries of
    its members including 40 million paid during the
    Korean, Vietnam and Persian Gulf conflicts.
  • Today AFBA insures over 520,000 lives with 35
    billion of insurance in force.

    include Emergency Services Personnel and First
    Responders throughout the country.

The Initial Offering
  • 5STAR GROUP LEVEL TERM LIFE INSURANCE

  • No War or Terrorism Exclusions
  • Coverage ON and OFF Duty
  • Payment of Emergency Death Benefit within 24
    hours.
  • Low Cost to Members
  • Portable Coverage
  • A One-Year 500 No-Cost Benefit
  • Available for those active members under Age 60
    who attend a meeting to learn about AFBAs 5Star
    Group Level Term Insurance.
  • Member must complete a special enrollment form.

No-Cost Option
  • All first responder personnel under age 59 are
    eligible for no-cost coverage of 500.
  • This amount is paid within 24 hours of
    notification of death (emergency death benefit)
    without a death certificate to the named
    beneficiary.
  • There is no cost to the sponsoring
    organization/association, departments, or
    individual for this program.

Eligibility for Group Level Term
  • The members of this group must be age 1-59 and a
    United States citizen.
  • The 5STAR GROUP LEVEL TERM program for first
    responder personnel is available in coverage
    amounts of 50,000 and 100,000.
  • Your spouse, through age 59, may also apply for
    coverage up to 100,000.

Eligibility for Group Level Term
  • Coverage continues to age 70 and cannot be
    canceled as long as all premiums are paid.
    Insurance premiums are based on gender, age and
    tobacco status.
  • Premiums will increase at each age divisible by 5
    after age 30 for males and age 35 for females the
    month following entry into a new age group.

Childrens Rider
  • Dependent children under age 21 (23 if a
    full-time student) are eligible.
  • All eligible children are covered under one rate.
  • Statement of Health questions must be answered
    for all children.
  • Ages 15 days to 6 months 1.00 per 1,000 unit
    for a maximum of 5 units (5,000).
  • Ages 6 months to 21 years (23 if full time
    student) 1.00 per 5,000 unit for a maximum of
    5 units (25,000).

Medical Requirements
  • Applicants under age 50 are asked to complete a
    statement of health on the enrollment form and
    those over the age of 50 are required to undergo
    a medical evaluation by a 5Star Life authorized
    paramedic or medical doctor.
  • In most cases, this will be accomplished in the
    privacy of your own home or office. A
    paramedical service will contact you for an
    appointment for the necessary tests and/or exams.
  • Physical exams, when required, will be arranged
    and paid by AFBA.

Enrollment
  • AFBA Representatives will schedule briefings with
    departments.
  • Individuals must specify a beneficiary for the
    No-Cost Benefit.
  • Individuals can elect to purchase additional
    coverage for themselves, their spouse, and their
    dependent children.
  • This is a 15 minute presentation that can be
    accomplished during regular department meetings.
